[摘要]目的:探讨烧伤后增生性瘢痕患者中铁代谢相关蛋白的表达情况。方法:组织标本均来自2016年-2018年石家庄市第
一医院烧伤整形外科住院患者。患者分三组:正常皮肤组、萎缩性瘢痕组、增生性瘢痕组。后两组患者分别采集瘢痕及其自
身正常皮肤标本。术中采集标本,应用电感耦合质谱分析仪测定三组标本中组织铁含量。免疫组织化学染色检测增生性瘢痕
中铁代谢相关蛋白转铁蛋白受体1 (transferrin receptor 1,TfR1)、铁蛋白(ferritin,FTL)、二价金属离子转运蛋白
1(divalent metal transporter 1,DMT1)、膜铁转运蛋白1(ferroportin 1,FPN1)等在增生性瘢痕中的表达。结果:
三组正常皮肤组织中组织铁含量比较差异无统计学差异(P >0.05)。增生性瘢痕组:增生性瘢痕组织比自身正常皮肤组织中
组织铁含量升高,差异有统计学意义(P <0.05)。免疫组化显示增生性瘢痕组中增生性瘢痕组织及自身正常皮肤组织中均有
铁代谢相关蛋白的表达,多表达于胞浆中,在表皮层表达较强。其中DMT1(±IRE)、FPN1以及FTL在增生性瘢痕组织中表达量
高于其自身正常皮肤组织(P <0.05);TfR1在增生性瘢痕组织中表达量却降低(P <0.05)。结论:增生性瘢痕组患者瘢痕
局部铁元素沉积过多,进而细胞内可能通过IRP/IRE系统调节铁的摄取和释放,而且推测主要依赖于TfR1以及FPN1的作用而
非DMT1(±IRE)。

Abstract:
Abstract: Objective To study the expression of iron metabolism related proteins in hypertrophic scars of burn patients.
Methods Tissue specimens were collected from inpatients in burn and plastic surgery department of Shijiazhuang First
Hospital from 2016 to 2018. The patients were divided into three groups: the normal skin group, the atrophic scar group and the
hypertrophic scar group. In the latter two groups, scars and normal skin specimens were collected. Specimens were collected
during the operation. Tissue iron content in three groups of specimens was determined by inductively coupled mass spectrum
analyzer. Immunohistochemical staining was used to detect the expression and localization of the iron metabolism related
protein oftransferrin receptor 1 (TfR1), ferritin (FTL), divalent metal transporter 1 (DMT1) and membrane iron transporter
1 (FPN1) in hypertrophic scars and its own normal skin specimens of the hypertrophic scar group. Results There was no
significant difference in iron content in normal skin tissue between the three groups (P >0.05). The iron content in hypertrophic
scar tissue in the hypertrophic scar group was higher than that in normal skin tissue (P <0.05). Immunohistochemistry showed
that iron metabolism related proteins were expressed in hypertrophic scar tissue and normal skin tissue of the hypertrophic scar
group. Most of them were expressed in the cytoplasm. The expression was strong in the epidermis. The expression of DMT1
(±IRE), FPN1 and FTL in hypertrophic scar tissue was higher than that in normal skin tissue (P <0.05). The expression of
TfR1 in hypertrophic scar tissue was decreased (P <0.05). Conclusion In hypertrophic scar patients, excessive iron deposition
in scar area may lead to intracellular regulation of iron uptake and release through IRP/IRE system, and it is presumed that the
role of TfR1 and FPN1 rather than DMT1 (±IRE) is mainly dependent on the role of TfR1 and FPN1.
