摘要:
[摘要]目的：探究自体富血小板凝胶在老年烧伤患者美容整复治疗中对创面愈合及肉芽组织特异性蛋白表达影响。方法：
选取2017年9月-2018年9月于笔者医院就诊的老年烧伤患者120例，依据治疗方式不同分为观察组和对照组，各60例。对
照组采用常规治疗，观察组在对照组基础上使用自体富血小板凝胶（autologous platelet-rich gel，APG），观察两组
患者创面愈合时间与创面愈合率，评定创面疼痛、瘢痕增生情况、并发症发生率及复发率，治疗后疗效及皮片/皮瓣成活
率，肿瘤坏死因子α（TNF-α）、白介素-1β（IL-1β）、IL-8、γ干扰素（INF-γ）等炎症因子变化，比较肉芽组织特
异性蛋白SDF-1α、VEGF、CXCR4蛋白表达情况。结果：观察组创面愈合时间、并发症发生率、复发率、疼痛视觉模拟评分
（Visual analogue scale，VAS）、温哥华瘢痕量表（Vancouver scar scale，VSS）及曼彻斯特瘢痕评定量表（Matching
assessment of photographs and scars，MAPS）评分均低于对照组，观察组创面愈合率、总有效率、皮片/皮瓣成活率高于
对照组，差异有统计学意义（P ＜0.05）。治疗后观察组TNF-α、IL-1β、IL-8和INF-γ水平降低，且低于对照组；观察组
SDF-1α、VEGF、CXCR4蛋白表达明显高于对照组，差异均有统计学意义（P ＜0.05）。结论：APG能够加速创面愈合，减少愈
合所需时间，促进肉芽组织特异性蛋白表达，对于老年烧伤患者美容整形修复具有促进作用，疗效可靠。

Abstract:
Abstract: Objective To investigate the effect of autologous platelet-rich gel on wound healing and granulation tissue-specific
protein expression in elderly burn patients. Methods 120 patients in our hospital from September 2017 to September 2018
were divided into two groups according to the random numerical table method. The patients in the control group were treated
with routine therapy and the patients in the observation group were treated with autologous platelet-rich gel (APG), on the basis
of the control group. The wound healing time, wound healing rate, wound pain, scar hyperplasia, complications and recurrence
probability, curative effect and survival rate of skin graft and skin flap were observed in the two groups, and the wound healing
time, wound healing rate, wound pain, scar hyperplasia, complications and recurrence probability were compared between two
groups. Inflammatory factors such as tumor necrosis factor-α (TNF-α), interleukin-1β (IL-1β) and IL-8, interferon -γ(INF-γ)
were changed to compare the expression of SDF-1α, VEGF, CXCR4 protein in granulation tissue. Results The wound healing
time, complication rate, recurrence rate, VAS score, VSS and MAPS score of the observation group were lower than those of
the control group, the wound healing rate, total effective rate and skin/flap survival rate of the observation group were higher
than those of the control group, the differences were statistically significant(P ＜0.05). After treatment, the levels of TNF -α, IL-
1β, IL-8 and INF -γ in the observation group were lower than those in the control group, and the expressions of SDF-1α, VEGF
and CXCR4 in the observation group were significantly higher than those in the control group(P ＜0.05). Conclusion APG can
accelerate wound healing, reduce healing time and promote the expression of granulation tissue-specific protein.
