摘要:
[摘要]目的：探讨益生菌制剂辅助治疗慢性牙周炎对IL-17、RANKL、OPG、RANKL/OPG水平的影响和美学效果观察。方法：
选取沧州市人民医院120例牙周慢性牙周炎患者为研究对象。根据患者病情程度参照Armitage推荐标准，将患者分度：轻、
中、重度，各40例，并随机分为对照组和观察组。对照组：行常规基础治疗洁治、刮治和根面平整；观察组：在对照组治疗
基础上每日给予益生菌制剂辅助治疗。所有患者在接受治疗前1周、治疗后6周、治疗后3个月均完成龈沟液提取及唾液样本
收集。比较两组患者治疗总有效率及治疗前、治疗后6周、3个月龈沟液IL-17、RANKL、OPG、RANKL/OPG及TNF-α、IL-1β水
平，并对两组患者治疗前后牙周美学指标龈乳头指数（PI）和龈乳头高度（PH）进行比较。结果：观察组患者治疗总有效率
(96.67%)明显高于对照组(86.67%)，组间比较差异有统计学意义(χ2=3.927, P =0.048)；治疗后，两组患者龈沟液IL-17、
RANKL、RANKL/OPG及TNF-α、IL-1β水平呈下降趋势，OPG水平呈上升趋势，治疗后3个月观察组患者龈沟液IL-17、RANKL、
RANKL/OPG及TNF-α、IL-1β水平明显低于对照组，OPG水平明显高于对照组，组间比较差异均有统计学意义(P ＜0.05)。两
组患者PI和PH均呈上升趋势，治疗后6周、治疗后3个月PI和PH水平均明显高于治疗前，且观察组明显高于治疗组，组间比较
差异均有统计学意义(P ＜0.05)。结论：益生菌制剂辅助治疗不同程度慢性牙周炎疗效显著，可有效改善患者龈沟液炎症相
关因子的水平，提升牙周组织美学效果，值得在不同严重程度牙周病临床治疗中应用。

Abstract:
Abstract: Objective To explore the effects of probiotics preparation on the levels of IL-17, RANKL, OPG, RANKL/OPG and
the aesthetic effects of chronic periodontitis. Methods The study selected 120 patients with periodontal chronic periodontitis in
Zhangzhou People’s Hospital. Patients were indexed according to the Armitage recommendation: 40 cases of mild, moderate,
and severe. 120 patients were randomly assigned to the control and observation groups. The control group underwent routine
basic treatment for scaling, scaling, and root planing. The observation group was given daily adjuvant treatment with probiotics
on the basis of the treatment of the control group. All patients received gingival crevicular fluid extraction and saliva sample
collection at 1 week before treatment, 6 weeks after treatment, and 3 months after treatment. The total effective rate of treatment
was compared between the two groups. The levels of IL-17, RANKL, OPG, RANKL/OPGI, TNF-α and IL-1β in gingival
crevicular fluid were compared before treatment, 6 weeks and 3 months after treatment. The differences in periodontal aesthetic
parameters, nipple index (PI) and nipple height (PH) between the two groups were compared. Results The total effective
rate of treatment in the observation group (96.67%) was significantly higher than that the control group (86.67%) (χ2=3.927,
P =0.048). After treatment, the levels of IL-17, RANKL, RANKL/OPGI, TNF-α and IL-1β in the gingival crevicular fluid of
the two groups showed a downward trend, and the OPG level showed an upward trend. After 3 months of treatment, the levels
of RANKL, RANKL/OPGI, TNF-α and IL-1β were significantly lower than those of the control group, and the OPG level was
significantly higher than that of the control group (P ＜0.05). Both PI and PH showed an upward trend in both groups. The PI
and PH levels were significantly higher than those before treatment at 6 weeks after treatment and 3 months after treatment, and
the observation group was significantly higher than the treatment group (P ＜0.05). Conclusion The application of probiotics
in the treatment of chronic periodontitis of different degrees is effective, which can significantly improve the level of gingival
crevicular fluid-related factors and improve the aesthetic effect of the damaged periodontium.It is worthy of clinical treatment of
periodontal disease with different severity application.
