[摘要]目的:观察紫杉醇对准分子激光上皮下角膜磨镶术(Laser epithelial keratomileusis,LASEK)后家兔角膜后表面
形态变化的影响。方法:选取27只健康新西兰大白兔纳入研究,建立兔眼LASEK模型,给予双眼-10D的LASEK手术治疗,随
机分为3组,每组9只兔(18只眼),对照方式选择自身对照法,A组术中使用紫杉醇,B组术中使用丝裂霉素C(MMC),C组
使用平衡盐液(BSS)作为阴性对照组,比较三组术后角膜后表面地形、角膜后表面前凸指标(Diff)、角膜后表面规则指
数(Lrreg),并观察3组角膜上皮下雾状混浊(Haze)分级。结果:A组与B组术后1周、术后1个月岛形所占比例明显高于C
组,差异有统计学意义(P <0.05);A组与B组比较差异无统计学意义(P >0.05)。三组术后3个月角膜后表面地形比较差
异无统计学意义(P >0.05)。三组术后1周、术后1个月、术后3个月Diff比较差异无统计学意义(P >0.05)。术后1周与术
后3个月,三组Lrreg比较差异无统计学意义(P >0.05);术后1个月,A组与B组Lrreg明显小于C组(P <0.05),A组与B组
比较差异无统计学意义(P >0.05)。A组与B组术后1周、术后1个月Haze分级明显优于C组(P <0.05),A组与B组比较差异
无统计学意义(P >0.05);三组术后3个月Haze分级比较差异无统计学意义(P >0.05)。结论:紫杉醇可有效抑制LASEK术
后家兔角膜Haze形成,减少对角膜后表面形态的影响,效果与MMC相当。

Abstract:
Abstract: Objective To observe the effects of paclitaxel on changes of corneal posterior surface morphology in rabbits after
laser subepithelial keratomileusis (LASEK). Methods 27 healthy New Zealand white rabbits were enrolled in the study.
LASEK model of rabbit eyes was established and LASEK was given to the double eyes for -10D. The rabbits were randomly
divided into three groups, with 9 rabbits (18 eyes) in each group. Self-control method was chosen for contrast, group A was
given paclitaxel during operation, and group B was given mitomycin C (MMC) during operation, and group C was given
balanced salt solution (BSS) as negative control group. The postoperative corneal posterior surface topography, corneal posterior
surface lordosis index (Diff), and corneal posterior surface rule index (Lrreg) were compared among the three groups, and the
Haze grading of corneal epithelium were observed in the three groups. Results The proportion of island shape in group A and
group B was signifi cantly higher than that in group C at 1 week and 1 month after operation (P <0.05). There was no signifi cant
difference between group A and group B (P >0.05). There was no signifi cant difference in the corneal posterior surface
topography at 3 months after operation among the three groups (P >0.05). There was no signifi cant difference in Diff
among the three groups at 1 week, 1 month and 3 months after operation (P >0.05). There was no signifi cant difference
in Lrreg among the three groups at 1 week and 3 months after operation (P >0.05). At 1 month after operation, the Lrreg
in group A and group B was signifi cantly smaller than that in group C (P <0.05). There was no signifi cant difference
between group A and group B (P >0.05) .The Haze grading in group A and group B at 1 week and 1 month after operation
was better than that in group C (P <0.05), and there was no signifi cant difference between group A and group B (P >0.05).
There was no signifi cant difference in Haze grading among the three groups at 3 months after operation (P >0.05). Conclusion
Paclitaxel can effectively inhibit the formation of corneal Haze in rabbits after LASEK, reduce the effects on corneal posterior
surface morphology, and its effect is equivalent to MMC.
