[摘要]目的:研究负压封闭引流(Vacuum sealing drainage,VSD)联合牛碱性成纤维细胞生长因子(Basic fibroblast
growth factor,bFGF)对四肢深Ⅱ度烧伤创面的治疗效果。方法:选择2018年4月-2019年3月笔者医院收治的80例四肢深Ⅱ
度烧伤患者,分别采用常规治疗(对照组,n =20),负压封闭引流治疗(治疗组A,n =20),牛碱性成纤维细胞生长因子治
疗(治疗组B,n =20)和负压封闭引流联合牛碱性成纤维细胞生长因子治疗(治疗组C,n =20)。观察比较四组患者创面大体
情况、创面愈合时间、创面愈合率、创面分泌物细菌培养情况和瘢痕增生情况。结果:经不同处理10d和21d后,治疗组A、
B和C创面改善情况明显优于对照组,且治疗组C情况改善最好;治疗组A、B和C创面愈合时间均明显低于对照组,治疗组C愈
合时间最短,差异均具有统计学意义(P <0.05)。10d、14d和21d后,治疗组A、B和C创面愈合率明显高于对照组,且10d、
14d后治疗组C创面愈合率最高(P<0.05),但21d后治疗组A、B和C创面愈合率无差别(P>0.05)。治疗组A、B和C患者愈
后1个月、2个月、4个月瘢痕颜色评分明显低于对照组,治疗组A、B和C患者愈后2个月、4个月瘢痕厚度、血管分布及柔软度
评分均明显低于对照组,且C组愈后4个月的各指标评分均明显低于治疗组A和B(P <0.05)。所有患者治疗后1d均无细菌感
染,但治疗10d、14d和21d后,治疗组A、B和C组细菌培养阳性病例数比例均明显低于对照组,治疗组C细菌培养阳性病例数
最低(P <0.05)。结论:负压封闭引流联合bFGF可有效加快四肢深Ⅱ度烧伤创面的愈合,改善愈后的瘢痕生长,减少创面
病原菌感染率,具有临床推广使用价值。

Abstract:
Abstract: Objective To study the therapeutic effect of vacuum sealing drainage (VSD) combined with basic fibroblast growth
factor (bFGF) on deep Ⅱ degree burn wounds of limbs. Methods From April 2018 to March 2019, our hospital 80 cases
of limb deep Ⅱ degree burn patients, respectively, using conventional treatment (the control group, n =20), vacuum sealing
drainage treatment (the treatment group A, n =20), basic fibroblast growth factor therapy (the treatment group B, n =20) and
vacuum sealing drainage combined with basic fibroblast growth factor therapy (the treatment group C, n =20). Observe and
compare the general situation of wound, wound healing time, wound healing rate, bacterial culture of wound secretion and
scar hyperplasia of the four groups. Results After 10d and 21d of different treatments, the wound surface improvement of the
treatment group A, B and C were significantly better than those of the control group, and the situation of the treatment group
C was the best. The wound healing time of the treatment group A, B and C were significantly lower than those of the control
group, and the healing time of the treatment group C were the shortest, the differences were statistically significant(P <0.05).
After 10d, 14d and 21d, the wound healing rate of the treatment group A, B and C were significantly higher than those of the
control group, and the wound healing rate of the treatment group C were the highest after 10d and 14d(P <0.05), but there were
no difference in the wound healing rate of the treatment group A, B and C(P >0.05). Treatment group A, B and C patients
improved after 1 month, 2 months, 4 months of scar color score significantly lower than the control group, treatment group
A, B and C patients after 2 months and 4 months, the scar thickness, blood vessel distribution and softness were significantly
lower than the control group, and group C after 4 months each index score were significantly lower than those of the treatment
group A and B (P <0.05). All the patients were free of bacterial infection one day after treatment, and the proportion of bacterial
culture positive cases in treatment group A, B and C was significantly lower than that in control group 10, 14 and 21 days
after treatment, and the number of bacterial culture positive cases in treatment group C was the lowest(P <0.05). Conclusion
Vacuum sealing drainage combined with bFGF can effectively accelerate the healing of deep Ⅱ degree burn wounds in the
limbs, improve the growth of scars after healing, and reduce the infection rate of pathogens on the wound surface.
