[摘要]目的:分析剖宫产后采用不同缝合方式进行腹壁切口缝合的效果。方法:将笔者医院2018年1月-2019年1月收治的行
剖宫产分娩的212例产妇作为研究对象,收集其临床资料,根据其分娩后腹壁切口缝合方式将其分为皮下脂肪层间断缝合组
(n =104)和皮下脂肪层不缝组(n =108),前者进行皮下脂肪层间断缝合,后者皮下脂肪层不缝,两组均进行皮内连续缝
合,对比两组腹壁切口缝合效果。结果:皮下脂肪层不缝组缝合时间、住院时间均短于皮下脂肪层间断缝合组,缝合所需成
本低于皮下脂肪层间断缝合组,差异有统计学意义(P <0.05)。术后1d和术后1个月,两组疼痛视觉模拟评分(Visual
analogue scale,VAS)、舒适度评分(Bruggman comfort score,BCS)比较差异无统计学意义(P >0.05);术后3d,
皮下脂肪层不缝组VAS评分低于皮下脂肪层间断缝合组(P <0.05),BCS评分高于皮下脂肪层间断缝合组(P <0.05)。
两组术后并发症发生率、切口甲级愈合率比较差异无统计学意义(P >0.05)。皮下脂肪层不缝组手术瘢痕满意率高于皮下
脂肪层间断缝合组(P <0.05)。结论:剖宫产后不进行皮下脂肪层缝合,能够增强产妇产后舒适度,提高其腹壁切口缝合
满意度。
Abstract:
Abstract: Objective To analyze effects of different suture methods for abdominal incision suture after cesarean section
delivery. Methods 212 puerpera who underwent cesarean section delivery and were admitted to the hospital from January 2018
to January 2019 were enrolled. Their clinical data were collected. According to different abdominal incision suture methods
after delivery, they were divided into the subcutaneous fat layer intermittent suture group (n =104) and the subcutaneous fat layer
non-suture group (n =108). The former underwent intermittent suture of subcutaneous fat layer, while the latter did not undergo
subcutaneous fat layer suture. Both groups underwent continuous intradermal suture. The abdominal incision suture effect
was compared between the two groups. Results The suture time and hospitalization time in the subcutaneous fat layer nonsuture
group were shorter than those in the subcutaneous fat layer intermittent suture group, the differences were statistically
significant (P <0.05). And suture cost was lower than that in the subcutaneous fat layer intermittent suture group (P <0.05).
At 1d and 1 month after surgery, there was no significant difference in visual analogue scale (VAS) or Bruggman comfort scale
(BCS) score between the two groups (P >0.05). At 3d after surgery, VAS score in the subcutaneous fat layer non-suture group
was lower than that in the subcutaneous fat layer intermittent suture group (P <0.05), while BCS score was higher than
that in the subcutaneous fat layer intermittent suture group (P <0.05). There was no significant difference in incidence of
incision complications or the rate of first-stage incision healing between the two groups (P >0.05). The satisfaction rate
of surgical scar in the subcutaneous fat layer non-suture group was higher than that in the subcutaneous fat layer intermittent
suture group (P <0.05). Conclusion After cesarean section delivery, not conducting subcutaneous fat layer suture can enhance
postpartum comfort, and improve satisfaction with abdominal incision suture of puerperae.
