[摘要]目的:探究黑素瘤患者组织中络氨酸激酶M2(PKM2)以及低氧诱导因子-1α(HIF-1α)表达水平变化及其临床意
义。方法:对2018年1月-2019年1月92例在笔者医院接受治疗的黑素瘤患者相关资料进行回顾性分析,患者均为手术治
疗,分别测定患者手术获取黑素瘤组织以及黑素瘤旁正常组织中PKM2、HIF-1α表达水平。不同组织PKM2、HIF-1α病
理染色结果分析,比较不同组织PKM2、HIF-1α表达水平差异,不同临床特征患者PKM2、HIF-1α表达水平差异,比较
不同预后患者PKM2、HIF-1α表达。结果:PKM2和HIF-1α分别在细胞质以及细胞核表达,阳性表达主要显示为棕黄色
以及黄色粒状物质,而阴性则显示为蓝色粒状物质。黑素瘤组织PKM2、HIF-1α表达阳性率高于黑素瘤旁正常组织,差异具
有统计学意义(P <0.05);淋巴结转移情况与分化程度不同患者黑素瘤组织中PKM2表达阳性率比较差异有统计学意义
(P <0.05);淋巴结转移情况、分化程度、肿瘤分期不同患者黑素瘤组织中HIF-1α表达阳性率比较差异有统计学意
义(P <0.05)。92例患者中位随访时间为10.34个月,随访期内失访3例,剩余89例患者中预后不良患者13例;预后良好组
患者黑素瘤组织PKM2、HIF-1α表达均明显低于预后不良组患者,差异有统计学意义(P <0.05)。结论:黑素瘤患者组织中
PKM2、HIF-1α高表达,其表达水平与患者病情进展以及预后情况关系密切,可用于评估患者病情并预测患者预后。

Abstract:
Abstract: Objective To investigate the changes in expression levels of pyruvate kinase M2 (PKM2) and Hypoxia inducible
factor-1α (HIF-1α) in tissues of patients with melanoma and their clinical significance. Methods The relevant data of 92
patients with melanoma treated in the hospital between January 2018 and January 2019 were analyzed retrospectively. All
patients were treated with surgery. The expression levels of PKM2 and HIF-1α in melanoma tissues and normal tissues adjacent
to melanoma were determined. Pathological staining results of PKM2 and HIF-1α in different tissues were analyzed. The
expression levels of PKM2 and HIF-1α in different tissues, in patients with different clinical features and different prognosis
were compared. Results PKM2 and HIF-1α were expressed in the cytoplasm and the cell nucleus, respectively. The positive
expression mainly showed brownish yellow and yellow granular substances, while the negative expression showed blue granular
substances. The positive expression rates of PKM2 and HIF-1α in melanoma tissues were higher than those in normal tissues
adjacent to melanoma (P <0.05). There were signifi cant differences in the expression of PKM2 in melanoma tissues of patients
with different conditions of lymph node metastasis and different differentiation degrees (P <0.05). There were significant
differences in the positive expression rate of HIF-1α in melanoma tissues of patients with different conditions of lymph node
metastasis, different differentiation degrees and different tumor stages (P <0.05). The median follow-up time of 92 patients
was 10.34 months, and 3 patients were lost to follow-up. Among the remaining 89 patients, 13 patients had poor prognosis.
The expression of PKM2 and HIF-1α in melanoma tissues of patients with good prognosis was signifi cantly lower than that in
patients with poor prognosis (P <0.05). Conclusion PKM2 and HIF-1α are highly expressed in patients with melanoma. The
expression levels are closely related to the patient’s disease progression and prognosis. They can be used to evaluate the patient’s
condition and predict the prognosis.
