[摘要]目的:探讨青少年安氏Ⅱ类1分类错牙合畸形患者经生理性支抗控制技术(Physiologic anchorage spee’s-wire
system,PASS)治疗后口腔颌面部软、硬组织的变化及相关性。方法:选取2015年1月-2018年1月在承德医学院附属医院诊治
的均采用PASS矫治技术治疗的32例安氏Ⅱ类1分类错牙合畸形青少年,且均上颌拔除两颗第一前磨牙、下颌拔除两颗第二前磨
牙的患者为研究对象。收集治疗前、后影像学资料,测量口腔颌面部软、硬组织指标,并进行统计学分析。结果:平均疗程
23.6个月,治疗前后上颌突度及唇倾的上下前牙得到明显改善(P <0.05);下颌骨的变化不明显,下颌平面角变化不明显
(P >0.05);面型突度及上、下唇的突度得到明显改善(P <0.05);∠NLA与∠SNA、∠U1-NA、U1-NA距、∠U1-SN呈现显
著相关;ULP与∠ANB、∠U1-NA、U1-NA(mm)显著相关。结论:PASS矫治技术能够不用任何附加的增强支抗的方式有效控制
支抗,有效改善青少年安氏Ⅱ类1分类错牙合畸形患者的面部软、硬组织,面部软组织会根据硬组织的变化而变化,但又不完
全一致。
Abstract:
Abstract: Objective To investigate the changes and correlations of oral and maxillofacial soft and hard tissues in adolescents
with Angle Class Ⅱ Division 1 malocclusion after treatment with Physiologic Anchorage Spee’s-wire System (PASS ).
Methods Thirty-two Adolescents who were diagnosed with Angle Class Ⅱ Division 1 malocclusion and treated with PASS at
the Affi liated Hospital of Chengde Medical university from January 2015 to January 2018. The patients with two fi rst premolars
removed from the maxilla and two second premolars removed from the mandible who were taken as the study subjects.
The image data before and after treatment were collected, and the soft and hard tissue indexes of the oral and maxillofacial
regions were measured and statistically analyzed. Results The mean course of treatment was 23.6 months. After treatment the
maxillary protrusion of patients were improved statistically signifi cant,the upper and lower anterior teeth were signifi cantly
improved(P <0.05). After treatment the mandibular plane angle did not change signifi cantly(P >0.05).After treatment
the facial protrusion of patients, and double lip protrusion obviously improved(P <0.05.Correlation analysis: ∠NLA showed
signifi cant correlation with ∠SNA, ∠U1-NA, U1-NA distance.ULP and ∠ANB, ∠U1-NA, U1-NA (mm) were signifi cantly
related. Conclusion The PASS technology can effectively control the anchorage without any additional way to enhance the
anchorage,and effectively improve the soft and hard tissues of adolescents with Angle Class Ⅱ Division 1 malocclusion. The
facial soft tissues will change according to the changes of the hard tissues but not exactly the same.
