摘要:
[摘要]目的:探究袋设计联合无张力缝合用于牙周加速成骨正畸(PAOO)的应用价值。方法:选取2016年4月-2019年4月在
西安交通大学第一附属医院口腔科拟行正畸治疗的患者80例,采用随机数表法分为改良组(40例)和对照组(40例),改
良组使用袋设计联合无张力缝合的PAOO,对照组使用传统PAOO,对比治疗前后牙槽骨指标,包括唇侧根尖 1/3 牙槽骨厚度
(AAHBT)、唇侧根中 1/3 牙槽骨厚度(AMHBT)、唇侧根冠 1/3 牙槽骨厚度(ACHBT)、牙根长度(RL)、唇侧牙槽嵴顶
的垂直向水平(AVBL);牙周指标,包括临床附着丧失(CAL)、牙龈萎缩深度(GRD)、探诊深度(PD);牙龈健康
指标,包括牙龈指数(GI)、龈沟出血指数(SBI)及患者治疗满意度。结果:治疗前两组牙槽骨形态指标无明显差异
(P >0.05),治疗12个月后两组ACHBT、AMHBT、AAHBT均较治疗前显著增加,AVBL较治疗前显著减小(P <0.05),其中
改良组ACHBT、AMHBT、AAHBT显著大于对照组,AVBL显著小于对照组(P <0.05),两组RL指标无明显改变(P >0.05);治
疗前两组牙周形态指标无明显差异(P >0.05),治疗后两组GRD、CAL均显著减少(P <0.05),其中改良组GRD、CAL显
著小于对照组(P <0.05),两组PD无明显改变(P >0.05);治疗前两组GI、SBI无明显差异(P >0.05),治疗后改良组
GI、SBI显著小于对照组(P <0.05);两组在正畸满意度指标上无明显差异(P >0.05)。结论:袋设计联合无张力缝合用
于PAOO可显著增加牙根冠骨量,同时有效改善治疗后牙周病损及牙龈健康情况。

Analysis of the Application Value of Pouch Design and Tension-free Wound Closure
in Periodontally Accelerated Osteogenic Orthodontics

Abstract:Objective To explore the value of pouch design and tension-free wound closure in periodontally accelerated
osteogenic orthodontics (PAOO). Methods From April 2016 to April 2019, 80 patients inDepartment of Stomatology, First
Affiliated Hospital of Xi’an Jiaotong University who planned to receive orthodontic treatment were selected and divided into
the modified group (40 cases) and the control group (40 cases) by random number table method. The modified group used
pouch design and tension-free wound closure PAOO, while the control group used traditional PAOO. Alveolar bone index
before and after treatment, including: anterior horizontal bone thickness of coronal one third at labial side (ACHBT), anterior
horizontal bone thickness of middle one third at labial side (AMHBT), anterior horizontal bone thickness of apical one third
at labial side (AAHBT), anterior vertical bone level (AVBL), root length (RL), periodontal examination indexes before and
after treatment, including: probing depth (PD), gingival recession depth (GRD), clinical attachment level (CAL), gingival
health indicators before and after treatment, including: gingival index (GI),sulcus bleeding index (SBI), and patient
satisfaction were compared. Results The alveolar bone index before treatment has no obvious difference (P >0.05), after
12 months of treatment, ACHBT, AMHBT and AAHBT in both two groups were significantly increased, while AVBL was
significantly decreased (P<0.05),which ACHBT, AMHBT and AAHBT in the modified group were significantly higher than
control group, and AVBL was significantly lower than control group (P <0.05), there was no significant change in RL index in
both two groups (P >0.05). There was no significant difference in periodontal morphological indexes between the two groups
before treatment (P > 0.05), GRD and CAL were significantly reduced in the two groups after treatment (P <0.05), which GRD
and CAL in the modified group were significantly lower than control group (P <0.05),there was no significant change in PD in
