[摘要]目的:探讨拱顶石穿支岛状皮瓣(Keystone design perforator island flap,KDPIF)设计原则及修复各类创面时
的方法改良。方法:应用KDPIF修复各部位创面11例,缺损面积为5.0cm×3.5cm~10.0cm×5.0cm,通过临床病例分析,探讨
该皮瓣的方法改良。结果:本组11例皮瓣均100%成活,伤口一期愈合,术后随访1年所有患者形态及功能满意。结论:KDPIF
在创面修复中具有设计简单、术时较短、血供可靠、适应性广、术后外观及功能好的特点;该皮瓣共有7种改良方式,根据
创面情况灵活应用改良KDPIF可使创面修复效果更佳。

Abstract:
Abstract: Objective To explore the design principle and the improvement strategies of keystone design perforator island
flap (KDPIF) in repairing various wounds. Methods KDPIF was used to repair wounds in 11 cases with the defect area from
5.0cm×3.5cm to 10.0cm×5.0cm, discussing the latest progress in clinical improvement strategy by analyzing clinical cases in
this paper. Results All the 11 flaps survived 100%, the wound healed in the first stage, and all the cases were satisfied with
morphology and function after 1 year of follow-up. At present 7 class of modified KDPIF are created. Conclusion KDPIF
has the characteristics of simple design, short operation time, reliable blood supply, wide adaptability, and good postoperative
appearance and function in wound repair. There are 7 ways to improve the flap, modified KDPIF can make the repair effect of
the wounds better.
