[摘要]目的:探讨皮肤镜、反射式共聚焦显微镜(RCM)在黄褐斑皮损评估中的应用价值。方法:收集黄褐斑患者127例,根据
近期皮损面积扩大、颜色加深情况将黄褐斑分为活动期和稳定期。用皮肤镜、RCM观察不同分期的黄褐斑皮损,分析临床分
期与皮损处血管的形态及数量、树突状黑素细胞形态及折光度、黑素数量和炎症细胞数量的相关性。结果:127例患者中,
进展期52例(占40.94%)、稳定期75例(占59.06%)。进展期患者RCM下树突状黑素细胞阳性率明显高于稳定期患者,差异
有统计学意义(χ2=103.52,P <0.05);皮肤镜下血管数量、RCM下炎症细胞进展期患者多于稳定期患者,差异有统计学意
义(均P <0.05);进展期及稳定期患者RCM下黑素细胞数量、皮肤镜下血管形态差异比较均无统计学意义(均P >0.05)。结
论:RCM、皮肤镜下分别观黄褐斑皮损处的树突状黑素细胞形态、炎症细胞数量及血管数量的改变可作为黄褐斑临床分期的
参考指标。

Abstract:
Abstract: Objective To investigate the application value of dermoscope and reflection confocal microscope (RCM) in the
assessment of chloasma lesions. Methods A total of 127 patients with chloasma were collected, and the chloasma was
divided into active period and stable period according to the recent enlargement of the lesion area and deepening of the color.
Dermoscopy and RCM were used to observe chloasma lesions at different stages, and the correlation between clinical stage
and vascular morphology and number, morphology and refraction of dendritic melanocytes, melanin number and number
of inflammatory cells at the lesion site was analyzed. Results Among the 127 patients, 52 patients in the progressive stage
accounted for 40.94% and 75 patients in the stable stage accounted for 59.06%.The positive rate of dendritic melanocytes under
RCM in patients with progressive stage was significantly higher than that in patients with stable stage, and the difference
was statistically significant(χ2=103.52,P <0.05).The number of blood vessels under dermoscope and the number of
patients with inflammatory cell progression under RCM were more than those at stable stage, and the difference was
statistically significant(P <0.05).There was no significant difference in the amount of melanin under RCM and the number
of blood vessels under dermoscope in patients with progressive stage and stable stage(P >0.05). Conclusion The changes
in the morphology of dendritic melanocytes, number of inflammatory cells and number of blood vessels in the skin lesions of
chloasma under RCM and dermoscopy can be used as reference indexes for the clinical stages of chloasma.
