摘要:
目的：探讨加盖岛状包皮皮瓣尿道成形术治疗患儿阴茎型尿道下裂的临床效果及护理体会。方法：选取2016年5月-2018年6月笔者医院收治的108例阴茎型尿道下裂患儿，根据手术方式不同将行加盖岛状包皮皮瓣尿道成形术的56例患儿作为观察组，行包皮岛状皮瓣尿道成形术治疗的52例患儿作为对照组，比较两组临床效果。结果：观察组患儿首次手术成功率为94.64%高于对照组的82.69%，差异有统计学意义（P＜0.05）。观察组下床活动时间、住院时间均短于对照组，差异有统计学意义（P＜0.05）。但两组手术时间比较差异无统计学意义（P＞0.05）。观察组并发症发生率为5.36%低于对照组的19.23%，差异有统计学意义（P＜0.05）。观察组患儿家属护理满意度评分高于对照组，差异有统计学意义（P＜0.05）。结论：阴茎型尿道下裂患儿采用加盖岛状包皮皮瓣尿道成形术治疗临床疗效好，可有效降低术后并发症 发生率、缩短下床活动时间及住院时间。
Abstract:
Objective To explore the clinical effect and nursing experience of urethroplasty with onlay island flap in the treatment of children with penile hypospadias. Methods A total of 108 children with penile hypospadias admitted to the hospital from May 2016 to June 2018 were enrolled. According to different surgical methodss 56 cases undergoing urethroplasty with onlay island flap were included into the observation group, while another 52 cases undergoing urethroplasty with preputial island flap were included into the control group. The clinical effects were compared between the two groups. Results The success rate of the first operation in the observation group was 94.64%, which was higher than that in the control group (82.69%), the difference was statistically significant (P＜0.05). The time of getting out of bed and hospitalization in the observation group were shorter than those in the control group (P＜0.05). However, there was no significant difference in operation time between the two groups (P＞0.05). The incidence of complications in the observation group was 5.36%, lower than that in the control group (19.23%, P＜0.05). The score of nursing satisfaction of family members in the observation group was higher than that in the control group (P＜0.05). Conclusion The clinical curative effect of urethroplasty with onlay island flap is good on children with penile hypospadias, which can effectively reduce the incidence of postoperative complications, shorten leaving bed time and hospitalization time.
