摘要:
目的:探讨种植体支抗应用于闭合式牵引埋伏牙的临床疗效及对口腔免疫炎性反应及微环境的影响。方法:选取2018年7月-2020年7月笔者医院收治的90例埋伏牙患者为研究对象,依据治疗方式的不同分为观察组(n=45,种植体支抗应用于闭合式牵引治疗)和对照组(n=45,闭合式开窗术联合传统固定治疗)。比较两组治疗后的临床疗效、口腔结构,评估两组治疗后牙周功能恢复情况,检测两组治疗前和治疗3个月后唾液中炎性指标肿瘤坏死因子-α(TNF-α)及白细胞介素-1β(IL-1β)、IL-6和免疫功能指标CD3+、CD4+、CD8+的含量,并依据数值计算CD4+/CD8+比值及基质金属蛋白酶-1(MMP-1)、基质金属蛋白酶-2(MMP-2) 和基质金属蛋白酶-9(MMP-9)的水平。结果:治疗后,观察组总有效率为93.33%,明显高于对照组的80.00%(P<0.05);观察组磨牙移位明显低于对照组,上中切牙凸距差和中切牙倾角差明显高于对照组(P<0.05);治疗后,观察组咀嚼功能、自我舒适度、牙齿稳定性、美观度以及满意度评分均明显高于对照组,差异有统计学意义(P<0.05)。治疗前,两组TNF-α、IL-1β和IL-6水平比较差异无统计学意义(P>0.05);治疗后,两组TNF-α、IL-1β和IL-6水平均上升,观察组明显低于对照组(P<0.05);治疗前,两组CD3+、CD4+ CD8+和CD4+/CD8+水平比较差异无统计学意义(P>0.05);治疗后,两组CD3+、CD4+和CD4+/CD8+水平均有所下降,CD8+水平升高,两组比较差异无统计学意义(P<0.05)。治疗前,两组MMP-1、MMP-2、MMP-9水平比较差异无统计学意义(P>0.05);治疗后,两组MMP-1、MMP-2、MMP-9水平均明显升高,观察组显著低于对照组,差异有统计学意义(P<0.05)。结论:种植体支抗应用于闭合式牵引埋伏牙治疗疗效显著,可有效改善牙周功能,降低炎症因子、改善基质金属蛋白酶水平,促进牙周微环境的平衡,有利于术后恢复。
Abstract:
Objective To investigate the clinical effect of implant anchorage applied in closed traction impacted teeth and its effect on oral immune inflammatory response and microenvironment. Methods A total of 90 patients with impacted teeth admitted to our hospital from July 2018 to July 2020 were selected as the research subjects, and were divided into observation group according to different treatment methods (n=45,Implant anchorage for closed traction therapy) and control group (n=45,Closed fenestration combined with conventional fixation). The clinical efficacy and oral cavity structure of the two groups after treatment was compared. The recovery of periodontal function in the two groups after treatment was evaluated. The levels of inflammatory markers tumor necrosis factor-α(TNF-α), interleukin-1β(IL-1β),IL-6 and immune function indexes CD3+, CD4+and CD8+ in saliva of both groups were detected before and 3 months after treatment, and the ratio of CD4+/CD8+and the levels of matrix metalloproteinase-1 (MMP-1), matrixmetalloproteinase-2 (MMP-2) and matrix metalloproteinase-9 (MMP-9) were calculated numerically. Results The total effective rate of observation group was 93.33%, which was significantly higher than that of control group (80.00%, P<0.05). The molar displacement in the observation group was significantly lower than that in the control group, and the difference of upper and central incisor convex pitch and central incisor Angle was significantly higher than that in the control group (P<0.05). After treatment, the chewing function, self-comfort, tooth stability, aesthetic degree and satisfaction scores of observation group were significantly higher than those of control group (P<0.05). Before treatment, the levels of TNF-α, IL-1β and IL-6 in the two groups were not significantly different (P>0.05). After treatment, the levels of TNF-α, IL-1β and IL-6 in the two groups were increased, and the observation group was significantly lower than the control group (P<0.05).Before treatment, the levels of CD3+, CD4+, CD8+and CD4+/CD8+were not significantly different between the two groups (P>0.05).After treatment, the levels of CD3+, CD4+and CD4+/CD8+were decreased, while the level of CD8+was increased, and the difference was significant between the two groups (P<0.05). Before treatment, the levels of MMP-1, MMP-2 and MMP-9 were not significantly different between the two groups (P>0.05). After treatment, the levels of MMP-1, MMP-2 and MMP-9 in the two groups were significantly increased, and the observation group was significantly lower than the control group (P<0.05). Conclusion The application of implant anchorage in the treatment of closed traction impacted teeth has a significant effect, which can effectively improve periodontal function, reduce inflammatory factors, improve the level of matrix metalloproteinase, promote the balance of the dental microenvironment, and facilitate postoperative recovery
