摘要:
目的:探讨负压创面治疗技术(Negative pressure wound therapy,NPWT)联合重组人碱性成纤维细胞生长因子 (Recombinant human basic fibroblast growth factor,rh-bFGF)持续冲洗治疗慢性创面的临床效果。方法:选取医院 烧伤整形外科2020年1月-12月收治的100例慢性创面患者为研究对象,随机分为A组和B组,每组各50例。A组采取NPWT联用生 理盐水冲洗治疗,B组为NPWT联用rh-bFGF溶液持续冲洗。比较两组肉芽组织覆盖率、疼痛评分、创面细菌阳性率及炎症指 标。结果:治疗后,两组疼痛评分、白细胞计数、C反应蛋白均降低,且B组低于A组,差异均有统计学意义(P<0.05)。 B 组新生肉芽组织覆盖率和创面愈合率明显高于A组,创面细菌阳性率低于A组,差异均有统计学意义(P<0.05)。结论: NPWT 联用rh-bFGF持续冲洗可降低创面感染率,促进一期肉芽组织新生,减少二期手术间隔时间,有一定临床应用价值。
Abstract:
Objective To observe the clinical effect of NPWT combined with rh-bFGF continuous fl ushing in the treatment of chronic wounds. Methods A total of 100 patients with chronic wounds treated in the department of burn and plastic surgery of our hospital from January to December 2020 were randomly divided into the group A (n=50) and the group B (n=50). The group A was treated with NPWT combined with normal saline, and the group B was treated with NPWT combined with rh- bFGF. The coverage rate of granulation tissue, pain score, bacteria positive rate and inflammation indexes of wound were compared between the two groups. Results After treatment, the pain score, WBC and C-reactive protein of the two groups decreased, and the group B were lower than the group A, the differences were statistically significant (P<0.05). The coverage rate of new granulation tissue and wound healing rate in the group B were significantly higher than those in the group A (P<0.05). The positive rate of wound bacteria in the group B was lower than that in the group A (P<0.05). Conclusion NPWT combined with rh-bFGF can reduce wound infection rate, promote granulation regeneration of primary wound, and reduce the interval between secondary operations, which has certain clinical value.
