摘要:
目的：探讨牙周-正畸结合治疗对重度牙周炎患者龈沟液基质金属蛋白酶-2（MMP-2）、白介素-1β（IL-1β）、核 因子κB受体活化因子配体/骨保护素（RANKL/OPG）水平的影响。方法：选取2018年6月-2020年6月笔者医院诊治的重度牙 周炎患者72例，以随机抽签法按1:1比例分为研究组和对照组，每组36例。对照组采取牙周组织再生术治疗，研究组采取牙 周-正畸结合治疗。对比两组临床疗效、治疗前后牙周指标［牙龈指数（GI）、探诊深度（PD）、龈沟出血指数（SBI）、 菌斑指数（PLI）、临床附着丧失（CAL）］水平、牙齿松动度、邻接点至牙槽骨嵴间距离（BC-CP）、龈乳头高度 （PH）、患牙前牙覆盖情况及龈沟液MMP-2、IL-1β、RANKL/OPG水平。结果：研究组总有效率（91.67%）高于对照组 （72.22%），差异有统计学意义（P ＜0.05）；与治疗前相比，两组治疗后GI、PD、SBI、PLI、CAL水平均明显降 低，且研究组低于对照组，差异有统计学意义（P ＜0.05）；两组治疗后牙齿松动度均低于治疗前，且研究组低于 对照组，差异均有统计学意义（P ＜0.05）；两组治疗后BC-CP、PH水平均高于治疗前，且研究组高于对照组，差异均有 统计学意义（P＜0.05）；与治疗前对比，两组治疗后前牙覆盖程度均得到明显改善，且研究组优于对照组，差异均有统计 学意义（P＜0.05）；两组治疗后龈沟液MMP-2、IL-1β、RANKL/OPG水平均明显降低，且研究组低于对照组，差异均有统计 学意义（P＜0.05）。 结论：牙周-正畸结合治疗能显著改善重度牙周炎患者切牙区美学效果，且能进一步下调龈沟液MMP2、IL-1β及RANKL/OPG水平，是治疗重度牙周炎的有效方法。
Abstract:
Objective To explore the combination of periodontal-orthodontic treatment on gingival crevicular fluid matrix metalloproteinase-2 (MMP-2), interleukin-1β(IL-1β),nuclear factor kappa B receptor activator ligand/osteoprotegerin in patients with severe periodontitis (RANKL/OPG) level of influence. Methods A total of 72 patients with severe periodontitis in our hospital from June 2018 to June 2020 were selected. They were divided into a study group and a control group at a ratio of 1:1 by a random lottery method, with 36 cases in each group. The control group was treated with periodontal tissue regeneration, while the study group was treated with periodontal-orthodontic treatment.The clinical efficacy, periodontal index [gingival index(GI),probing depth (PD),gingival crevicular bleeding index (SBI), plaque index (PLI), clinical attachment loss (CAL)] levels before and after treatment were compared between the two groups Looseness, distance between adjacent joints and alveolar bone crest (BC-CP), gingival papilla height (PH), anterior tooth coverage of the affected tooth, gingival crevicular fluid MMP-2, IL-1β, and RANKL/OPG levels. Results The total effective rate in the study group (91.67%) was higher than (72.22%) in the control group(P＜0.05). Compared with before treatment, the levels of GI, PD, SBI, PLI, and CAL in the two groups were significantly reduced after treatment, and the study group was lower than the control group (P ＜0.05).The tooth looseness of the two groups after treatment was lower than that before treatment, and the study group was lower than the control group (P＜0.05).After treatment,the BC-CP and PH levels of the two groups were higher than those before treatment, and the study group was higher than the control group (P＜0.05).Compared with before treatment, the coverage of the anterior teeth after treatment in the two groups improved significantly, and the study group was better than the control group (P＜0.05).After treatment, the levels of gingival crevicular fluid MMP-2, IL-1β,and RANKL/OPG decreased significantly in the two groups, and the study group was lower than the control group (P＜0.05). Conclusion The combined periodontal-orthodontic treatment can significantly improve the aesthetic effect of the incisor area in patients with severe periodontitis, and can further reduce the levels of MMP-2, IL-1β,and RANKL/OPG in gingival crevicular fluid, which is an effective method for the treatment of severe periodontitis.
