目的:通过锥形束计算机断层扫描(Cone-beam computed tomography,CBCT)分析新疆地区成人上颌前牙牙根的矢状位 置及各分类的分布情况。方法:回顾性分析100名研究对象上颌中、侧切牙及尖牙的CBCT矢状位图像,根据牙根在牙槽骨中 的位置分为颊侧型、中间型及腭侧型,并且测量牙根相对于牙槽骨长轴的角度。结果:大多数上前牙的牙根位于牙槽骨的 唇侧 (占76.0%),中间型占22.3%,腭侧型最少见,仅占1.7%。其中,中切牙唇侧型占73.0%、中间型占24.0%、腭侧型占 3.0% ,侧切牙唇侧型占68.0%、中间型占31.0%、腭侧型仅占1.0%,尖牙唇侧型占87.0%、中间型占12.0%、腭侧型仅占1.0%; 中切牙、侧切牙及尖牙的牙根矢状角度分别为(8.84±2.62)°,(14.52±3.74)°及(19.39±6.24)°,两两比较差异 均有统计学意义(P<0.05);不同牙根位置分型比较中,唇侧型(16.71±4.82)°显著大于中间型(6.36±2.60)°及腭 侧型(7.52±6.20)°,差异有统计学意义(P<0.05)。结论:中国新疆地区人群上颌前牙牙根多位于牙槽骨唇侧,唇侧 骨壁较薄,唇侧型尖牙的矢状角度最大,中间型尖牙矢状角度最小。
Abstract:
Objective To analyse the sagittal root position in maxillary anterior teeth in the alveolar bone using cone-beam computed tomography (CBCT)in the population from Xinjiang area. Methods CBCT sagittal images of central,lateral incisors and canines from 100 patients were retrospectively reviewed,and the root position in the alveolar bone was classified as labial, middle or palatal,then the angulation of the root axis and the alveolar bone axis was measured. Results A majority of the maxillary anterior teeth were positioned more labially within the alveolar bone accounted for 76%,the middle type accounted for 22.3%, and only 1.7% were positioned more palatally.Among them, the labial type of central incisor accounted for 73%, that of middle type 24%, that of palatal type only 3%, that of lateral incisor labial type 68%, that of middle type 31%, that of palatal type only 1%, that of incisor labial type 87%, that of middle type 12%, and that of palatal type only 1%;There were statistically significant differences in sagittal angulation between anterior teeth of labial type, middle teeth and palatal type (P <0.05), there was no significant difference in sagittal root angulation between the middle type and the palatal type (P>0.05), and there were significant differences in sagittal root angulation between the central incisors, lateral incisors and cusps (P<0.05);The sagittal root angulation of central incisors, lateral incisors and canines were (8.84±2.62)°,(14.52±3.74)°and (19.39±6.24)°, respectively, and the differences were statistically significant (P<0.05);The labial type (16.71±4.82)°was significantly higher than the middle type (6.36±2.60)°and the palatal type (7.52±6.20)°,and the difference was statistically significant(P<0.05). Conclusion A thinner labial alveolar bone overlying maxillary anterior teeth was usually present in the population from Xinjiang area with majority of the maxillary incisors was positioned more labially within the alveolar bone.The sagittal angulation of the labial type of canines was the largest, while the middle type of canines was the smallest.
