摘要:
目的:探讨超脉冲点阵CO2激光联合维芎瘢痕霜治疗面部烧伤后增生性瘢痕的临床疗效。方法:选取2016年6月-2020 年6月笔者科室收治的120例面部烧伤后增生性瘢痕患者为研究对象,随机分为治疗组与对照组,每组60例。对照组采用维 芎瘢痕霜进行治疗,治疗组采用超脉冲点阵CO2激光联合维芎瘢痕霜治疗。比较治疗前后两组的临床疗效、温哥华瘢痕量表 (Vancouver scar scale,VSS)评分、北卡罗纳大学瘢痕量表评分、视觉模拟量表(Visual analogue scale,VAS)评分、 血流灌注量及治疗期间的不良反应情况。结果:治疗后,对照组有8例复发,15例无效,总有效率为61.67%,治疗组有3例复发,6例无效,总有效率为85.00%,对照组总有效率低于治疗组,差异有统计学意义(P<0.05);两组VSS评分和北卡罗纳 大学瘢痕量表评分均低于治疗前,且治疗组VSS评分和北卡罗纳大学瘢痕量表评分均低于对照组(P<0.05);治疗3、6个月 后,两组VAS评分均低于治疗前,且治疗组VAS评分低于对照组(P<0.05);治疗3个月后,两组血流灌注量均高于治疗前, 且治疗组血流灌注量低于对照组(P<0.05);治疗6个月后,两组血流灌注量均低于治疗前和治疗后3个月,且治疗组血流 灌注量低于对照组(P<0.05);治疗期间两组不良反应发生情况比较,差异无统计学意义(P>0.05)。结论:超脉冲点阵 CO2 激光联合维芎瘢痕霜治疗烧伤后面部增生性瘢痕可提高临床疗效,减少瘢痕增生,减轻患者疼痛,改善血流灌注量,安全性好。
Abstract:
Objective To explore the clinical curative eff ect of ultra-pulse fractional CO2 laser combined with weixiong scar frost on hypertrophic scars after facial burns. Methods A total of 120 patients with hypertrophic scars after facial burns admitted to the hospital were enrolled between June 2016 and June 2020.They were randomly divided into treatment group and control group,60 cases in each group.The control group was treated with weixiong scar frost,while treatment group was additionally treated with ultra-pulse fractional CO2 laser.The clinical curative effect,scores of Vancouver scar scale (VSS),University of North Carolina Scar Scale and Visual analogue scale (VAS),and blood perfusion volume before and after treatment,and occurrence of adverse reactions during treatment were compared between the two groups. Results After treatment,there were 8 cases with recurrence and 15 cases with ineff ective treatment in control group,with total response rate of 61.67%.In treatment group,there were 3 cases with recurrence and 6 cases with ineff ective treatment,with total response rate of 85.00%. The total response rate in control group was lower than that in treatment group (P<0.05).After treatment,scores of VSS and the University of North Carolina Scar Scale in both groups were decreased,which were lower in treatment group than control group (P<0.05).After 3 and 6 months of treatment,VAS scores were lower than those before treatment in both groups,which were lower in treatment group than control group (P<0.05).After 3 months of treatment,blood perfusion volume was more than that before treatment in both groups,which was less in treatment group than control group (P<0.05).After 6 months of treatment,blood perfusion volume was less than before treatment and after 3 months of treatment in both groups,which was less in treatment group than control group (P<0.05).During treatment,diff erence in incidence of adverse reactions between the two groups was not statistically signifi cant,and the incidence in both groups was relatively low (P>0.05). Conclusion Ultra-pulse fractional CO2 laser combined with weixiong scar frost can improve clinical curative effect on hyperplastic scars after facial burns,alleviate scar hyperplasia and pain,and improve blood perfusion,with good safety.
