摘要:
目的:探讨牙周组织再生术(Periodontal tissue regeneration,GTR)联合口腔正畸治疗对慢性牙周炎患者口腔 功能及肿瘤坏死因子(Tumor necrosis factor-α,TNF-α)、C-反应蛋白(C-reactive protein,CRP)等炎症因子水平 的影响。方法:选取2018年1月-2020年12月笔者医院收治的慢性牙周炎患者作为研究对象,按随机数字表法分为观察组 (n=59)和对照组(n=59)。对照组患者给予GTR治疗,观察组患者给予GTR联合口腔正畸治疗,比较两组患者治疗前 后牙周状态指标,包括牙周探诊深度(Probing depth,PD)、附着水平(Attachment level,AL)及牙龈指数(Gingival index,GI);比较两组患者治疗前后炎症因子水平,包括血清白细胞介素-8(IL-8)、白细胞介素-6(IL-6)、TNF-α、 CRP 水平;比较两组临床疗效,记录治疗半年内两组不良反应发生率及牙周炎复发率。结果:治疗后,两组患者PD、AL、GI 较治疗前下降,且观察组低于对照组,差异有统计学意义(P<0.05);治疗后,两组患者IL-8、IL-6、TNF-α、CRP等炎 症因子水平较治疗前下降,且观察组低于对照组,差异有统计学意义(P<0.05)。观察组治疗有效率为96.61%,高于对照 组的76.27%,差异有统计学意义(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05);观察组复发率 为3.39%,低于对照组的13.56%,差异有统计学意义(P<0.05)。结论:给予慢性牙周炎患者GTR联合口腔正畸治疗效果显 著,可显著改善牙周状态,恢复口腔功能,减轻炎症水平,且复发率较低。
Abstract:
Objective To investigate the eff ects of periodontal tissue regeneration combined with orthodontic treatment on oral function and the levels of tumor necrosis factor (TNF-α), C-reactive protein (CRP) and other infl ammatory factors in patients with chronic periodontitis. Methods A total of 118 patients with chronic periodontitis in our hospital from January 2018 to December 2020 were selected as the research objects. They were randomly divided into observation group (n =59) and control group (n=59). Patients in the control group were treated with periodontal tissue regeneration, and patients in the observation group were treated with periodontal tissue regeneration combined with orthodontic treatment. Indexes of periodontal status including probe depth (PD), attachment level (AL) and gingival index (GI) were compared between the two groups before and after treatment. The levels of infl ammatory factors, including serum interleukin-8 (IL- 8), interleukin-6 (IL-6), TNF-α and CRP, were compared between the two groups before and after treatment. The clinical effi cacy of the two groups were compared, and the adverse reactions and recurrence rate of periodontitis in the two groups were recorded within half a year. Results After treatment, the PD, AL and GI of the two groups were signifi cantly decreased compared with those before treatment, and the observation group was lower than the control group (P <0.05). After treatment, the levels of IL-8, IL-6, TNF-α, CRP and other infl ammatory factors in the two groups were signifi cantly decreased compared with those before treatment, and the levels in the observation group were lower than those in the control group (P <0.05). The eff ective rate of observation group was 96.61%, which was higher than that of control group (76.27%) (P <0.05). There was no signifi cant diff erence in the incidence of adverse reactions between the two groups (P >0.05). The recurrence rate of the observation group was 3.39%, lower than that of the control group (13. 56%) (P<0.05). Conclusion Periodontal tissue regeneration combined with orthodontic treatment for patients with chronic periodontitis can signifi cantly improve the periodontal status, restore oral function, reduce the level of infl ammation, and the recurrence rate is low.
