摘要:
目的:分析社会性康复计划对重症烧伤后自闭症患儿情绪障碍和身心康复的影响。方法:选取2016年5月-2019年9月 笔者医院收治的重症烧伤后自闭症患儿97例,随机分为观察组、对照组,分别49例、48例,对照组予以常规护理干预,观察 组在此基础上采用社会性康复计划干预,以孤独症行为量表(Autism behavior checklist,ABC)、能力低下儿童评定量表 (Pediatric evaluation of disability inventory,PEDI)、Gesell婴幼儿发展量表(Gesell development diagnosis scale,GDDS)、儿童孤独症评定量表(Child autism rating scale,CARS)评估两组患儿症状、社交与自理能力、中枢神 经系统发育情况、依赖程度,同时记录患儿负面情绪[儿童焦虑性情绪障碍自评量表(Screen for child anxiety related emotional disorders,SCARED)、儿童抑郁性情绪障碍自评量表(Depression self-rating scale for children, DSRSC )]、情绪[焦虑自评量表(Self-rating anxiety scale,SAS)、抑郁自评量表(Self-rating depression scale,SDS)]变化,采用比里奇家庭生活质量量表(the Beach centre family quality of life scale ,BCFQOLS)评估 家庭生活质量。结果:观察组干预后ABC量表评分(35.86±4.34)分,低于对照组(38.31±3.86)分,而PEDI评分 (67.10±6.93)分、Gesell评分(104.90±9.67)分高于对照组(62.69±7.58)分、(94.48±9.64)分,差异均有统计 学意义(P<0.05)。观察组干预后CARS依赖程度48.98%低于对照组75.00%,差异有统计学意义(P<0.05)。干预后观察 组患儿SCARED评分(16.69±1.71)分、DSRSC评分(12.67±1.25)分,较对照组(19.79±1.81)分、(18.10±2.21)分 低,差异有统计学意义(P<0.05)。观察组家长SAS评分(31.92±3.98)分、SDS评分(33.12±3.36)分也低于对照组 (34.21±3.36)分、(38.94±3.77)分,差异有统计学意义(P<0.05)。观察组干预后BCFQOLS总分及家人互动、情绪需 求、生活需求维度得分均高于对照组,差异有统计学意义(P<0.05)。结论:社会性康复计划应用于重症烧伤后自闭症患 儿中,可明显改善其症状,提高社交与自理能力,促进患儿中枢神经系统发育,减少其依赖程度及负性情绪,降低家长情绪 障碍,提高家庭生活质量,值得在临床推广实践。
Abstract:
Objective To analyze the impact of social rehabilitation program on emotional disorder and physical and psychological rehabilitation of autistic children after severe burns. Methods Between May 2016 and September 2019, 97 autistic children after severe burns from the hospital were enrolled in this study and randomized into two groups: the observation group (49 cases) and the control group (48 cases). Patients in the control group and the observation group were given routine nursing intervention and social rehabilitation program intervention, respectively. Symptoms, social activity, selfcare ability, development of central nervous system and dependence degree of the two groups of children were evaluated using the ABC, PEDI, Gesell and CARS. Changes in children’s negative emotions (SCARED, DSRSC) and parental emotions (SAS, SDS) were recorded. Meanwhile, BCFQOLS was used to assess family life quality. Results After intervention, the ABC score of the observation group was (35.86±4.34), lower than (38.31±3.86) of the control group, while the PEDI score (67.10±6.93) and Gesell score (104.90±9.67) were higher than (62.69±7.58) and (94.48±9.64) of the control group, the differences were statistically signifi cant (P <0.05). After intervention, the dependence degree in the CARS of observation group was lower than that of the control group (48.98% vs 75.00%, P<0.05). After intervention, SCARED score (16.69±1.71) and DSRSC score (12.67±1.25) in the observation group were lower than those in the control group (19.79±1.81) and (18.10±2.21), the diff erences were statistically signifi cant (P <0.05). SAS score (31.92±3.98) and SDS score (33.12±3.36) of parents in the observation group were also lower than those in the control group (34.21±3.36) and (38.94±3.77), the diff erences were statistically significant (P <0.05). After intervention, the total score of BCFQOLS, family interaction, emotional needs and life needs in the observation group were higher than those in the control group (P<0.05). Conclusion This study suggests that social rehabilitation program applied to children with autism after severe burns can signifi cantly alleviate their symptoms, improve their social and self-care ability, promote the development of their central nervous system, reduce their dependence and negative emotions while reducing parents’ emotional disorder and improving family life quality. Therefore, the social rehabilitation program is worthy of clinical practice.
