摘要:
目的：探讨光子嫩肤仪联合调Q激光治疗黄褐斑的近远期疗效。方法：选取2018年1月-2020年1月笔者科室确诊的118 例女性黄褐斑患者，采用简单随机分组法分为观察组和对照组，每组59例。对照组采用调Q激光治疗，每月1次，共治疗6个 月；观察组在对照组基础上联合光子嫩肤仪治疗，其中光子嫩肤使用频率为每2个月1次。对比两组患者的疗效，采用SOFT 5.5皮肤检测仪对治疗前后皮肤性质进行检测，采用VISIA皮肤分析仪评估棕色斑特征计数，计算其下降率；对比治疗前后患 者面部黄褐斑皮损面积与严重程度指数（Melasma area severity index，MASI）评分，记录两组治疗期间面部潮红、轻度 水肿、灼痛感等不良反应发生情况；随访12个月，记录两组患者治疗后6个月、9个月、12个月的复发率。结果：治疗后，观 察组有效率为96.61%，高于对照组的83.05%（P＜0.05）；治疗后，观察组色素斑、纹理、皱纹、毛孔及紫质评分均低于 对照组（P＜0.05）；且观察组皮肤水分、弹性评分高于对照组，pH值、油脂评分低于对照组（P＜0.05）；治疗后，观察 组棕色斑计数下降率显著高于对照组，观察组MASI评分显著低于对照组，差异均有统计学意义（P＜0.05）；治疗期间， 两组不良反应发生率比较差异无统计学意义（P＞0.05）；随访12个月发现，观察组复发率显著低于对照组，差异有统计学 意义（P＜0.05）。结论：光子嫩肤仪联合调Q激光治疗黄褐斑疗效显著，皮损改善明显，复发率低，值得临床推广应用。
Abstract:
Objective To explore the short-term and long-term curative eff ect of intense pulsed light combined with Q-switched laser on patients with chloasma. Methods A total of 118 female patients with chloasma confi rmed in the hospital were enrolled between January 2018 and January 2020. According to simple random grouping method, they were divided into observation group and control group, 59 cases in each group. The control group was treated with Q-switched laser (once/month for 6 months), while observation group was treated with IPL (once/two months) on basis of control group. The curative effect between the two groups was compared. The skin properties were detected by SOFT 5.5 skin detector before and after treatment. The feature counts of brown spots were evaluated by VISIA skin analyzer to calculate the decrease rate. The scores of melasma area severity index (MASI) before and after treatment were compared. The occurrence of adverse reactions (facial fl ushing, mild edema, causalgia) during treatment in both groups was recorded. After 12 months of follow-up, recurrence rates at 6, 9 and 12 months in both groups were recorded. Results After treatment, response rate in observation group was higher than that in control group (96.61% vs 83.05%) (P ＜0.05). After treatment, pigmented spots, texture, wrinkle, pores and poryphyrins precipitation in observation group were lower than those in control group (P ＜0.05), scores of skin moisture and elasticity were higher than those in control group, pH value and grease score were lower than those in control group (P＜0.05). After treatment, decrease rate of brown spot counts in observation group was significantly higher than that in control group (P＜0.05), and MASI score was significantly lower than that in control group (P＜0.05). There was no significant difference in the incidence of adverse reactions between the two groups during treatment (P＞0.05). After 12 months of follow-up, recurrence rate in observation group was significantly lower than that in control group (P＜0.05). Conclusion The curative effect of IPL combined with Q-switched laser is significant on chloasma patients. The skin problems are improved significantly and recurrence rate is low.
