摘要:
目的:比较1 540 nm非剥脱点阵激光和2 940 nm铒像素激光治疗面部皮肤光老化的效果。方法:选择2020年6月-2021年 6 月笔者医院就诊的120例皮肤光老化就医者为研究对象,按随机数字表法分为点阵激光组(n=59)和像素激光组(n=61), 其中点阵激光组接受1 540 nm非剥脱点阵激光治疗,像素激光组接受2 940 nm铒像素激光治疗,比较治疗后两组就医者皮肤红 斑及黑色素、表皮角质层含水量和经表皮水分丢失量、皱纹和斑点情况,以及并发症发生情况。结果:治疗后,两组皮肤红 斑及黑色素评分较治疗前显著降低,且点阵激光组低于像素激光组,差异均有统计学意义(P<0.05);治疗后,两组就医 者表皮角质层含水量较治疗前显著提高(P<0.05),经表皮水分丢失量较治疗前显著降低(P<0.05),且两组比较差异有 统计学意义(P<0.05);治疗后,两组就医者皱纹和斑点评分较干预前显著降低,且点阵激光组显著低于像素激光组,差 异有统计学意义(P<0.05);点阵激光组并发症发生率(5.08%)低于像素激光组(9.84%),但两组比较差异无统计学意 义(P>0.05)。结论:1 540 nm非剥脱点阵激光和2 940 nm铒像素激光均能减少皮肤红斑、黑色素、皱纹、斑点,防止经表 皮水分丢失,提高肌肤含水量,达到改善皮肤光老化的效果,但两者相比,1 540 nm非剥脱点阵激光效果更明显,安全性更 高,值得临床推广应用。
Abstract:
Objective To compare the effects of 1 540 nm non-ablative fractional laser and 2 940 nm erbium pixel laser on improving skin photoaging. Methods 120 patients with skin photoaging who were treated in the hospital between June 2020 and June 2021 were selected as the research subjects. According to the random number table method, the patients were randomly divided into fractional laser group (n=59) and pixel laser group (n=61). The fractional laser group received 1 540 nm non- ablative fractional laser treatment, and the pixel laser group received 2 940 nm erbium pixel laser treatment. After treatment, the skin erythema and melanin, water content of stratum corneum epidermidis and transepidermal water loss, wrinkles and spots and occurrence of complications were compared between the two groups. Results After treatment, the skin erythema index and melanin index of the two groups were significantly decreased compared with those before treatment (P <0.05), and the scores of fractional laser group were significantly lower than those of pixel laser group (P<0.05). The water content of stratum corneum epidermidis of the two groups of patients after treatment was significantly increased compared to before treatment (P<0.05) while the transepidermal water loss was signifi cantly reduced compared to before treatment (P <0.05), and there were signifi cant diff erences between the two groups after treatment (P <0.05). After treatment, the scores of wrinkles and spots of the two groups were signifi cantly lower than those before intervention (P <0.05), and the scores of fractional laser group were signifi cantly lower than those of pixel laser group (P <0.05). The incidence rate of complications were lower in fractional laser group than that in pixel laser group (5.08% vs 9.84%) (P>0.05). Conclusion Both 1 540 nm non-ablative fractional laser and 2 940 nm erbium pixel laser can reduce the skin erythema, melanin, wrinkles and spots, prevent the transepidermal water loss, increase skin moisture content, and achieve the eff ect of improving skin photoaging, but 1 540 nm non-ablative fractional laser has more obvious effect and higher safety, thus it is worthy of clinical promotion.
