摘要:
目的:探究新型单纤维可吸收胶原蛋白缝合材料在桡骨远端骨折患者创面闭合中的应用效果及对创面瘢痕形成的影 响。方法:选取2021年4月-2022年9月收治的200例桡骨远端骨折患者,按随机数表法将其分为研究组与对照组各100例。 研究组采用新型单纤维可吸收胶原蛋白缝合线缝合伤口,对照组采用PGA/PGLA可吸收缝合线缝合伤口。比较两组伤口愈合 情况、疼痛程度、创面瘢痕形成情况、炎症指标[肿瘤坏死因子α(Tumor necrosis factor α,TNF-α)、白细胞介素6 (Interleukin 6,IL-6)、C反应蛋白(C-reactive protein,CRP)]及血清血管内皮生长因子(Vascular endothelial growth factor,VEGF)、胰岛素样生长因子1(Insulin-like growth factor 1,IGF-1)、骨形态发生蛋白2(Bone morphogenetic protein 2,BMP-2)含量变化。结果:研究组术后伤口甲、乙、丙级愈合率分别为95.00%、5.00%、0.00%, 优于对照组的85.00%、14.00%、1.00%(P<0.05);术后3 d、5 d,研究组疼痛评分低于对照组(P<0.05);两组术后 TNF-α 、IL-6、CRP低于术前,且研究组低于对照组(P<0.05);两组术后VEGF、IGF-1、BMP-2含量高于术前,且研究组高 于对照组(P<0.05);研究组创面瘢痕发生率16.00%低于对照组的28.00%(P<0.05)。结论:新型单纤维可吸收胶原蛋白 缝合材料在桡骨远端骨折患者创面闭合中的应用效果较好,可明显提升伤口愈合等级,减少创面瘢痕,促进伤口恢复。
Abstract:
Objective To explore the application eff ect of the new single-fi ber absorbable collagen suture material in the wound healing for patients with distal radius fractures and its infl uences on wound scars. Methods A total of 200 patients with distal radius fractures admitted to the hospital were enrolled between April 2021 and September 2022.According to random number table method,they were divided into study group (100 cases) and control group (100 cases).The study group was given the new single-fiber absorbable collagen suture,while control group was given PGA/PGLA absorbable suture to close wounds. The healing status,pain status,formation of wound scars and changes in levels of inflammatory indexes [tumor necrosis factor α (TNF-α),interleukin 6 (IL-6),C-reactive protein (CRP)],serum vascular endothlial growth factor (VEGF),insulin-like growth factor 1 (IGF-1) and bone morphogenetic protein 2 (BMP-2) were compared between the two groups. Results After surgery,wound healing rates of grades A,B and C in study group were 95.00%,5.00% and 0.00%,better than those in control group (85.00%,14.00%,1.00%) (P <0.05).At 3 d and 5 d after surgery,pain scores in study group were lower than those in control group (P <0.05).After surgery,levels of TNF-α,IL-6 and CRP in both groups were decreased,which were lower in study group than control group (P <0.05).After surgery,levels of VEGF,IGF-1 and BMP-2 in both groups were increased,which were higher in study group than control group (P <0.05).The incidence of wound scars in study group was lower than that in control group (16.00% vs 28.00%) (P<0.05). Conclusion The application eff ect of the new single-fi ber absorbable collagen suture material is good in the wound healing for patients with distal radius fractures,which can signifi cantly improve the grading of wound healing,reduce wound scars and promote recovery.
