目的:探讨浓缩生长因子纤维蛋白膜联合Bio-Oss骨粉修复拔牙术后牙槽骨缺损的疗效。方法:选择2018年1月-2020 年1月笔者医院收治的因拔除下颌阻生第三磨牙致第二磨牙远中骨缺损患者76例,予以随机数字表法分为对照组(Bio-Oss 骨粉植入,38例)和观察组(CGF膜联合Bio-Oss骨粉植入,38例)。手术后3 d复诊观察患者术后疼痛、开口度情况,比较 两组患者术后1周创面愈合情况以及术前、术后1个月和3个月牙槽骨密度、牙槽嵴高度及牙槽嵴宽度,记录术后不良反应。 结果:对照组术后3 d VAS评分(3.35±0.47)分,观察组为(2.67±0.42)分,对照组开口度(2.75±0.46)cm,观察组 (2.68±0.51)cm,两组比较差异均无统计学意义(P>0.05)。术后1周,观察组创面愈合情况好于对照组,差异有统计学 意义(P<0.05);术后1个月、术后3个月,观察组牙槽骨密度、牙槽嵴高度、牙槽嵴宽度均高于对照组,差异有统计学意 义(P<0.05)。对照组术后不良反应发生率23.68%,高于观察组的5.26%,差异有统计学意义(P<0.05)。结论:浓缩生 长因子纤维蛋白膜联合Bio-Oss骨粉在第二磨牙远中骨缺损修复中发挥着重要作用,值得临床推广应用。
Abstract:
Objective To investigate the eff ect of concentrated growth factor fi brin membrane combined with bio OSS bone powder in repairing alveolar bone defect after tooth extraction. Methods 76 patients with distal bone defect of the second molar caused by extraction of mandibular impacted third molar in the author’s hospital from January 2018 to January 2020 were randomly divided into 38 cases in the control group (Bio-Oss bone powder implantation) and 38 cases in the observation group (CGF membrane combined with Bio-Oss bone powder implantation). They were followed up three days after operation to observe the postoperative pain and opening. The wound healing 1 week after operation was compared between the two groups. Alveolar bone mineral density, alveolar ridge height and alveolar ridge width before operation, 1 month and 3 months after operation. The postoperative adverse reactions were recorded. Results The 3-day VAS score of the control group was (3.35±0.47) points, that of the observation group was (2.67±0.42) points, the opening degree of the control group was (2.75±0.46) cm, and that of the observation group was (2.68±0.51) cm. There was no signifi cant diff erence (P >0.05). The wound healing of the observation group was better than that of the control group one week after operation (P<0.05). The alveolar bone mineral density, alveolar ridge height and alveolar ridge width in the observation group at 1 month and 3 months after operation were signifi cantly higher than those in the control group (P <0.05). The incidence of postoperative adverse reactions in the control group was 23.68%, and the incidence of postoperative adverse reactions in the observation group was 5.26%, the diff erence was statistically signifi cant(P <0.05). Conclusion Concentrated growth factor fi brin membrane combined with Bio-Oss bone powder plays an important role in the repair of distal bone defect of the second molar, which is worthy of clinical promotion.
