摘要:
目的：探讨负压封闭引流技术（Vacuum sealing drainage,VSD）结合专项疼痛管理对烧伤整形患者疼痛缓解及创面 恢复的影响。方法：选取2018年1月-2020年12月在笔者医院接受游离植皮术治疗的烧伤患者90例，简单随机分组为观察组 （n=45）与对照组（n=45）。两组患者术后均给予VSD干预，对照组在VSD干预的基础上给予常规术后护理，观察组在对照 组基础上联合应用专项疼痛管理，持续干预直至患者出院。比较并记录两组患者护理疗效、心理状态、疼痛程度，记录肉 芽生长时间、创面愈合时间、住院费用、并发症和满意度。结果：护理后，观察组疗效为95.56%高于对照组的82.22%（P ＜0.05）。护理后，观察组肉芽生长时间、创面愈合时间和住院费用数值低于对照组（P＜0.05）。术前，两组患者疼痛视 觉模拟评分（Visual analogue scale,VAS）比较差异无统计学意义（P＞0.05）；术后第1、3、5天，观察组VAS评分均低 于对照组（P＜0.05）。护理前，两组患者焦虑自评量表(Self-rating anxiety scale,SAS)、抑郁自评量表（Self-rating depression scale，SDS）与匹茨堡睡眠质量指数量表(Pittsburgh sleep quality index,PSQI)评分比较差异均无统计学意 义（P＞0.05）；护理后两组SAS、SDS评分均低于护理前，且观察组低于对照组（P＜0.05）。护理后，两组PSQI评分均低于 护理前，且观察组低于对照组（P＜0.05）。观察组总并发症发病率低于对照组（P＜0.05）。护理后，观察组患者手术外 观、心理护理、健康教育、服务态度及对手术操作专业性的满意度评分均高于对照组（P＜0.05）。结论：对烧伤整形患者 应用VSD结合专项疼痛管理可有效缓解疼痛，促进创面恢复，提高患者满意度。
Abstract:
Objective To explore the eff ects of vacuum sealing drainage (VSD) combined with special pain management on pain relief and wound recovery in patients undergoing burn plastic surgery. Methods A total of 90 patients with burns undergoing free skin graft in the hospital were enrolled between January 2018 and December 2020.According to simple random grouping method,they were divided into observation group (n=45) and control group (n=45).Both groups underwent VSD after surgery. On this basis,control group was given routine nursing,while observation group was given special pain management on basis of control group.All were continuously nursed till discharge.The nursing effi cacy,psychological status and pain level between the two groups were compared.The granulation growth time,nursing,wound healing time,hospitalization cost,complications and treatment satisfaction were recorded. Results After nursing,the curative eff ect of observation group (95.56%) was higher than that of control group (82.22%) (P ＜0.05). After nursing,the values of granulation growth time,wound healing time and hospitalization cost in observation group were lower than those in control group (P ＜0.05). Before surgery,there was no signifi cant diff erence in visual analogue scale (VAS) between the two groups (P ＞0.05). The VAS scores of the observation group were lower than those of the control group on the 1st,3rd and 5th day after surgery (P＜0.05). Before nursing,self-rating anxiety scale (SAS),self-rating depression scale (SDS),There was no signifi cant diff erence between SDS and pittsburgh sleep quality Index (PSQI) (P ＞0.05). The scores of SAS and SDS in the two groups after nursing were lower than those before nursing,and the observation group was lower than the control group (P ＜0.05). After nursing,PSQI scores in both groups were lower than before nursing,and observation group was lower than control group (P ＜0.05). The incidence of total complications in observation group was lower than that in control group (P＜0.05). After nursing,the surgical appearance,psychological nursing,health education,service attitude and satisfaction scores of surgical operation in observation group were higher than those in control group (P＜0.05). Conclusion The application of VSD combined with special pain management can eff ectively relieve pain,promote wound recovery and improve treatment satisfaction in patients undergoing burn plastic surgery.
