摘要:
目的：观察彩色多普勒超声（Color doppler ultrasound,CDU）技术在小腿穿支皮瓣修复足部及踝关节周围难愈性创 面中的应用效果。方法：选择2016年1月-2021年11月笔者医院收治的60例足部及踝关节周围复杂难愈性创面患者，所有患者 均在CDU辅助检查下采用小腿穿支皮瓣进行修复，观察患者手术情况、临床疗效、皮瓣肿胀程度、足踝功能。结果：所有患 者均顺利完成手术，穿支定位符合率达90.00%，皮瓣存活率为96.67%，患者手术时间、皮瓣切取时间、手术总费用分别为 （145.36±10.78）min、（42.68±3.52）min、（1.49±0.35）万元。术后6个月，所有患者临床疗效优良率为96.67%，足 踝功能评分明显优于术前（P＜0.05）。结论：CDU技术辅助下小腿穿支皮瓣修复足部及踝关节周围难愈性创面效果确切，皮 瓣存活率较高，足踝功能恢复良好。
Abstract:
Objective The aim of this study was to observe the application eff ect of color Doppler ultrasound (CDU) in repairing foot and peri-ankle refractory wounds with perforator flaps from the lower leg. Methods This study included 60 patients with foot and peri-ankle refractory wounds who underwent CDU-guided repair with perforator fl aps from the lower leg in the author’s hospital from January 2016 to November 2021.Surgical conditions,clinical effi cacy,degree of skin fl ap swelling,foot and ankle function were observed. Results All the patients successfully completed the operation,the perforator positioning coincidence rate was 90.00%,and the fl ap survival rate was 96.67%. The operation time,fl ap harvesting time,and total operation cost were (145.36±10.78) min,(42.68±3.52) min,and (1.49±0.35) thousand yuan,respectively. At 6 months after operation,the excellent and good rate of clinical efficacy was 96.67%,and the foot and ankle function score was significantly better than that before operation (P ＜0.05). Conclusion CDU-guided repair with perforator fl aps from the lower leg is eff ective in the treatment of foot and peri-ankle refractory wounds,and the survival rate of fl aps is high.Besides,the treatment method can well restore foot and ankle function.
