摘要:
目的:探讨上颌单颗前牙缺失患者即刻种植时机对牙槽骨吸收量及美学效果的影响。方法:选取2019年10月-2021年 10 月于笔者医院口腔修复科预行种植义齿修复的上颌单颗前牙缺失患者84例,按照即刻种植时机不同分为对照组(n=42)和 观察组(n=42)。对照组采用延期种植即刻修复术,观察组采用即刻种植即刻修复术。比较两组临床疗效,采用红色美学指 数(Pink esthetic score,PES)和白色美学指数(White esthetic score,WES)评估两组软组织美学效果,比较两组骨 吸收相关指标、并发症及患者满意度。结果:观察组治疗有效率高于对照组(P<0.05);修复后3个月,观察组PES评分和 WES 评分均高于对照组、牙槽骨吸收值低于对照组(P<0.05);修复后6个月,两组PES评分和WES评分均降低、牙槽骨吸收 值均增加,且观察组PES评分和WES评分均高于对照组、牙槽骨吸收值低于对照组(P<0.05);而两组牙槽嵴宽度、唇侧骨 板厚度和并发症总发生率比较,差异均无统计学意义(P>0.05);修复后6个月,观察组整体美观、附着高度、咀嚼功能评 分均高于对照组(P<0.05),两组种植体色泽比较,差异均无统计学意义(P>0.05)。结论:上颌单颗前牙缺失患者行即 刻种植即刻修复效果显著,可有效恢复患者牙龈软组织美学效果,骨吸收量较低,患者满意度较高。
Abstract:
Objective To investigate the eff ect of immediate implant timing on alveolar bone resorption and aesthetic eff ects in patients with maxillary single anterior tooth loss. Methods A total of 84 patients with missing maxillary single anterior teeth who underwent pre-implant denture restoration in the Department of Prosthodontics of the author’s hospital from October 2019 to October 2021 were selected and divided into control group (n=42) and observation group (n=42). The control group was treated with delayed implantation and immediate repair, and the observation group was treated with immediate implantation and immediate repair. The clinical effi cacy of the two groups was compared. The red aesthetic index (PES) and the white aesthetic index (WES) were used to evaluate the soft tissue aesthetic eff ect of the two groups, and the bone resorption-related indicators, complications and patient satisfaction were compared between the two groups. Results The eff ective rate of treatment in the observation group was higher than that in the control group (P <0.05). Three months after repair, the PES and WES scores of the observation group were higher than those of the control group, and the alveolar bone resorption value of the observation group was lower than that of the control group (P<0.05). 6 months after repair, the PES and WES scores of the two groups were decreased, and the alveolar bone resorption values were increased, and the PES and WES scores of the observation group were higher than those of the control group, and the alveolar bone resorption values were lower than those of the control group (P<0.05). There was no signifi cant diff erence in the width of alveolar ridge, the thickness of the labial bone plate and the total complication rate between the two groups (P >0.05). After 6 months of repair, the overall appearance, attachment height and masticatory function scores of the observation group were higher than those of the control group (P<0.05). There was no signifi cant diff erence in implant color between the two groups (P >0.05). Conclusion Immediate implant restoration in patients with missing maxillary anterior teeth has a signifi cant eff ect, which can eff ectively restore the aesthetic eff ect of the patients’ gingival soft tissue, with low bone resorption and high patient satisfaction.
