目的：研究真皮脂肪组织游离移植美容方案的构建及其在颌面部缺损凹陷患者中应用。方法：选取2018年1月-2022年 1 月在笔者医院就诊的颌面部缺损凹陷的患者90例，按照随机数字表法分为观察组和对照组，每组45例。对照组患者采用颗 粒脂肪填充术；观察组给予真皮脂肪组织游离移植。比较两组患者术后1 d、术后1周、术后4周及术后12周的有效率；比较 两组患者治疗6个月后的瘢痕情况。比较两组患者满意度情况、不良反应发生率及需要二次手术率。结果：观察组术后1 d及 术后1周有效率比较差异无统计学意义（P＞0.05）。术后4周及术后12周，观察组有效率高于对照组（P＜0.05）。治疗前， 两组患者温哥华瘢痕量表（Vancouver scar scale，VSS）及石溪瘢痕评估量表（Stony brook scar evaluation scale， SBSES ）比较差异无统计学意义（P＞0.05）。治疗后6个月，观察组VSS评分低于对照组（P＜0.05），SBSES评分高于对照组 （P＜0.05）。治疗后3个月，观察组患者VAS评分高于对照组（P＜0.05）。治疗期间，两组患者不良反应发生率比较差异无 统计学意义（P＞0.05）。治疗后3个月，观察组患者二次手术率低于对照组（P＜0.05）。结论：真皮脂肪组织游离移植治 疗颌面部缺损具有良好的临床疗效，且患者满意度高，术后不良反应少。
Abstract:
Objective To study the construction of cosmetic scheme of free dermal adipose tissue transplantation and its application in maxillofacial defect patients. Methods A total of 90 patients seen in the author’s hospital from January 2018 to January 2022 were selected and divided into observation group and control group according to the randomized number table method, with 45 patients in each group. The control group used facial fat fi lling. The observation group was given a cosmetic scheme of free dermal adipose tissue transplantation.Both groups were compared at 1 day, 1 week, 4 weeks, and 12 weeks thereafter. After 6 months of treatment, the scarring condition was compared between the two groups. Patient satisfaction, incidence of adverse reactions and need of secondary surgery were compared between the two groups. Results The diff erence was different 1 day or 1 week after surgery(P ＞0.05). At 4 weeks and 12 weeks later,the observation group was more eff ective than the control group(P ＜0.05). Before treatment, Vancouver scar scale(VSS) score and stony brook scar evaluation scale(SBSES) score were diff erent between the two groups(P ＞0.05) .Six months after treatment, the VSS score was lower than the control group(P＜0.05), and the SBSES score was higher than the control group(P＜0.05). Three months after treatment, the VAS score of the observation group was higher than the control group(P＜0.05). There was no diff erence in the incidence of adverse reactions between the two groups during the treatment period(P ＞0.05). Three months after treatment, the rate of the observation group was lower than that of the control group(P＜0.05). Conclusion Dermal adipose tissue free transplantation has a good clinical eff ect in the treatment of maxillofacial defects, with high patient satisfaction and few postoperative adverse reactions.
