摘要:
目的：观察局部皮瓣联合异体巩膜移植修复眼睑及其周围皮肤缺损的临床效果。方法：选取2019年1月-2021年1月笔 者医院收治的90例眼睑恶性肿瘤切除术后眼睑及周围皮肤缺损患者，采用随机数字表法分为观察组和对照组，各45例，对照 组采用异体巩膜移植修复，观察组在此基础上联合局部皮瓣修复，比较两组修复效果，修复前后睑裂长度、眼睑高度差值以 及修复后6个月瘢痕宽度、瘢痕颜色、弹性、平整度、眼睑形态恢复程度、外观满意度评分，以及并发症发生情况。结果： 观察组修复总有效率高于对照组（P＜0.05）；两组修复后睑裂长度、眼睑高度均较修复前降低（P＜0.05），且观察组低于 对照组（P＜0.05）；观察组修复后6个月瘢痕宽度小于对照组，瘢痕颜色、弹性、平整度、眼睑形态恢复程度、外观满意度 评分均高于对照组（P＜0.05）；观察组术后上睑下垂、睑缘切痕等并发症总发生率低于对照组（P＜0.05）。结论：局部皮 瓣联合异体巩膜移植修复眼睑及其周围皮肤缺损疗效显著且并发症少，术后眼睑形态及美学效果良好。
Abstract:
Objective To observe the effects of local skin flap combined with allogeneic sclera transplantation on repairing eyelid and surrounding skin defects. Methods A total of 90 patients with eyelid and surrounding skin defects after resection of malignant eyelid tumor treated in the author’s hospital from January 2019 to January 2021 were selected and divided into observation group and control group by random number table method, with 45 cases in each group. The control group was repaired with allogeneic sclera transplantation, and the observation group was additionally repaired with local skin flap on this basis. The repair eff ects, diff erences of palpebral fi ssure length and eyelid height before and after repair, scar width, scar color, elasticity, fl atness, eyelid shape recovery score and appearance satisfaction at 6 months after repair and occurrence of complications were compared between the two groups. Results The total effective rate of repair in observation group was higher than that in control group (P＜0.05). The palpebral fi ssure length and eyelid height after repair were reduced compared with those before repair (P ＜0.05), and the diff erences in observation group were lower than those in control group (P ＜0.05). The scar width in observation group at 6 months after repair was lower than that in control group while the scores of scar color, elasticity, fl atness, eyelid shape recovery and appearance satisfaction were higher than those in control group (P＜0.05). The total incidence rate of postoperative complications such as ptosis and eyelid margin notching in observation group was lower compared to control group (P＜0.05). Conclusion Local skin flap combined with allogeneic sclera transplantation has significant efficacy, few complications and good postoperative eyelid shape and aesthetic effect in repairing eyelid and surrounding skin defects.
