目的:分析羊膜移植术与结膜瓣移植术后治疗翼状胬肉的临床疗效。方法:选取2020年1月-2022年12月在安徽省庐江 县人民医院的眼科收治的80例原发性翼状胬肉手术后的患者作为研究对象,以治疗方式为基础分为观察组与对照组,每组各 40 例,观察组接受原发性翼状胬肉术后游离结膜瓣移植术,对照组接受原发性翼状胬肉术后羊膜移植术。于术后比较两组患 者角膜上皮创面愈合情况、疼痛情况、角膜地形变化、翼状胬肉大小对地形恢复的影响。结果:经两组患者角膜上皮愈合评 分显示,两组间差异无统计学意义(P>0.05);术后1周后,两组患者的视觉模拟评分(Visual analogue scale,VAS)评 分结果显示,观察组VAS评分优于对照组(P<0.05);术后1周,观察组复发率低于对照组(P<0.05);术后1个月,角膜 中央光带处的曲率角膜顶点与光心之间的距离、平均角膜表面形状因子、角膜不规则性测量均小于术前,且观察组小于对照 组;中央光学区的角膜曲率高于术前,且观察组高于对照组,差异有统计学意义(P<0.05)。结论:原发性翼状胬肉切除联 合游离结膜瓣移植术或羊膜移植术对于原发性翼状胬肉都有很好的治疗效果,两种手术方式角膜上皮愈合程度没有明显的差 异,且疼痛评定指数对比无差异,两组术后复发率均得到有效改善,但观察组的复发率要优于对照组,且视觉模拟评分优于 对照组。
Abstract:
Objective To analyze the clinical eff ect of amniotic membrane transplantation and conjunctival fl ap transplantation in the treatment of pterygium. Methods 80 patients with primary pterygium who underwent surgery in the ophthalmology department of Lujiang County People’s Hospital in Anhui Province from January 2020 to December 2022 were selected as the research subjects. Based on the treatment method, they were divided into an observation group and a control group, with 40 patients in each group. The observation group received primary pterygium surgery and free conjunctival fl ap transplantation, while the control group received primary pterygium surgery and amniotic membrane transplantation. Compare the effects of corneal epithelial wound healing, pain, corneal topography changes, and pterygium size on terrain recovery between two groups of patients after surgery. Results According to the corneal epithelial healing score of two groups of patients, there was no signifi cant diff erence between the two groups (P >0.05). After one week of surgery, the VAS scores of the two groups of patients showed that the observation group had better VAS scores than the control group (P<0.05). One week after surgery, it was observed that the recurrence rate between the two groups was lower in the observation group than in the control group (P<0.05). One month after operation, the distance between the curvature corneal apex and the optical center, the average corneal surface shape factor and corneal irregularity at the central corneal band were all smaller than before operation, and the observation group was smaller than the control group. The corneal curvature in the central optical zone was higher than that before operation, and that in the observation group was higher than that in the control group, with statistical signifi cance (P <0.05). Conclusion Both primary pterygium excision combined with free conjunctival fl ap transplantation and primary pterygium excision combined with amniotic membrane transplantation have good therapeutic eff ects on primary pterygium. There is no signifi cant diff erence in the degree of corneal epithelial healing between the two surgical methods, and there is no diff erence in pain assessment index. The postoperative recurrence rates of both groups have been eff ectively improved, but the recurrence rate of the observation group is better than that of the control group. And the visual simulation score was better than the control group.
