[摘要]目的：调查第二恒磨牙错位的相关因素，为口腔正畸临床诊断和治疗提供参考。方法：随机抽取500例符合条件的正
畸患者，男196例，女304例，平均年龄（21.54±3.7）岁。根据模型对第二恒磨牙错位的发病率进行调查。并从中选取120
例（40例第二恒磨牙正常萌出，40例上颌第二恒磨牙错位萌出，40例下颌第二恒磨牙错位萌出）正畸患者进一步研究。对
120例样本的记存模型、曲面断层片及头颅侧位片进行测量、记录和分析。研究的变量使用独立样本t 检验和χ2检验进行统
计学分析。结果：第二恒磨牙错位的发病率为52% 。上颌第二恒磨牙错位与上颌第一恒磨牙处牙槽弓宽度、ANB角、上颌后
牙弓段拥挤度存在相关性（P ＜0.05）；下颌第二恒磨牙错位与下颌第一、二恒磨牙间牙槽弓宽度、SNB角、ANB角及下颌后
牙弓段拥挤度存在相关性（P ＜0.05）。第二恒磨牙错位与SN/MP、第二恒磨牙牙冠宽度、第三恒磨牙的存在无明显相关性
（P ＞0.05）。结论：较窄的第一恒磨牙间牙槽弓宽度、较小的ANB角和拥挤的上颌后段牙弓是引起上颌第二恒磨牙错位的因
素。较窄的下颌磨牙间牙槽弓宽度、较小的SNB角、较大的ANB角及拥挤的下颌牙弓后段是导致下颌第二恒磨牙错位的因素。

Abstract:
Abstract: Objective To investigate the related-factors of the second permanent molars dislocation, in order to provide
reference for the clinical diagnosis and treatment of orthodontics. Motheds 500 patients with orthodontic conditions were
randomly selected, male 196 cases, female 304 cases, average age （21.54±3.7） years. The incidence of dislocation of
second permanent molars was investigated according to the model.120 patients(40 patients with normal eruption of the second
molars,40 patients with ectopic eruption of maxillary second molars,40 patients with ectopic eruption of lower second molars )
were selected for further study. The recording model, the curved fault slice and the skull side slice were measured, recorded and
analyzed in 120 samples. Studied variables were analyzed statistically by independent T-tests and chi-square test. Results The
incidence of dislocation of the second permanent molar was 52%.The dislocation of the maxillary second permanent molar was
significantly correlated with the width of the alveolar arch,∠ANB,and the crowding of the maxillary posterior arch (P ＜0.05).
The dislocation of the mandibular second permanent molar was significantly correlated with the width of the alveolar arch, the
∠SNB, the ∠ANB and the crowding of the mandibular posterior arch (P ＜0.05). The dislocation of the second permanent
molar was not significantly correlated with the SN/MP, the second permanent molar crown width and the third permanent molar
(P ＞0.05). Conclusion The width of the alveolar arch, the smaller ∠ANB and the crowded maxillary posterior segment were
the factors causing dislocation of the maxillary second molar. The narrow mandibular molar arch width, smaller ∠SNB, larger
∠ANB and crowded mandibular posterior segment were the factors causing dislocation of the mandibular second molar。
