[摘要]目的：探讨削痂植皮术对深度烧伤患者瘢痕外观及愈合时间的影响，并分析相关影响因素。方法：选取2016年
1月-2019年1月笔者医院收治的176例深度烧伤患者作为研究对象，根据手术方式不同分为对照组（n =70）和观察组
（n =106）。对照组给予保痂肉芽创面植皮术，观察组则给予削痂植皮术。对比两组手术情况、植皮情况、并发症发生情况
和临床疗效，采用Logistic回归模型分析探讨影响深度烧伤患者临床疗效的关键因素。结果：两组手术时间和术中出血量
比较，差异无统计学意义（P ＞0.05）。与对照组比较，观察组创面愈合时间缩短，一次性植皮成活率升高，并发症总发生
率明显下降，临床疗效更佳，其总有效率明显升高，差异均有统计学意义（P ＜0.05）。经Logistic回归分析得知，烧伤程
度、治疗方式和并发症发生情况是影响深度烧伤患者临床疗效的关键因素（P ＜0.05）。结论：与传统保痂肉芽创面植皮术
比较，削痂植皮术治疗深度烧伤患者创面愈合时间缩短，植皮成活率升高，并发症发生率降低，临床总有效率显著提高，值
得推广。

Abstract:
Abstract: Objective To explore the effect of escharectomy and skin grafting on the scar appearance and healing time of
deep burn patients, and analyze the relevant influencing factors. Methods 176 patients with deep burn wounds admitted to
our hospital from January 2016 to January 2019 were selected as the research objects, and they were divided into the control
group (n =70) and the observation group (n =106) according to the different operation methods. The control group was given
escharectomy and granulation skin grafting, while the observation group was given escharectomy and skin grafting. The
operation conditions, skin grafting, complications and clinical effects were compared between the two groups. The key factors
influencing the clinical effect of patients with deep burn wound were analyzed by Logistic regression model. Results There
was no significant difference between the two groups in operation time and bleeding volume (P ＞0.05). Compared with the
control group, the wound healing time of the observation group was shortened, the survival rate of one-time skin grafting
was increased, the total incidence of complications was significantly reduced, the clinical effect was better, the total
effective rate was significantly increased, the differences were statistically significant (P ＜0.05). Logistic regression
analysis showed that the degree of burn, treatment and complications were the key factors affecting the clinical efficacy
of patients with deep burn (P ＜0.05). Conclusion Compared with the traditional escharectomy and granulation skin grafting,
escharectomy and skin grafting can shorten the wound healing time, increase the skin grafting rate, reduce the incidence of
complications, and significantly improve the clinical efficiency, which is worthy of promotion.
