摘要:
目的：精细缝合结合局部皮瓣修复术 眶周组织损伤患者伤口愈合后瘢痕情况的影响。方法：选取笔者医院2018年6 月-2021年6月收治的89例面部外伤患者，分为研究组46例对照组43例，对照组予传统缝合技术修复，对照组予精细缝合 结合局部皮瓣修复术修复，比较两组临床疗效、瘢痕程度（温哥华瘢痕量表，Vancouver scar scale，VSS）和瘢痕美 观度（患者和观察者瘢痕评估量表，Patient and observer scar assessment scale，POSAS)，及眼睑组织是否完全复 位，有无睑外翻和睑球分离。结果：干预后，研究组治疗后的有效率（97.83%）明显高于对照组（86.05%），差异具 有统计学意义（P＜0.05）。治疗后两组VSS总分较治疗前均降低，研究组（2.08±0.62）较对照组（3.66±0.53） 下降更明显（P＜0.05）。治疗后两组POSAS总分均降低，研究组（11.33±2.15）较对照组（15.26±2.09）下降更 明显（P＜0.05）。修复后，研究组眼睑完全复位患者比例（43/46）较对照组（30/43）有统计学差异（P＜0.05）， 研究组睑外翻例数（2/46）较对照组（5/43）无统计学差异（P＞0.05），研究组睑球分离例数（4/46）较对照组 （8/43）无统计学差异（P＞0.05）。结论：精细缝合结合局部皮瓣修复术在眼眶骨折的修复中，可有效减少愈合后的瘢痕 程度，提高瘢痕美观度，使创面修复后更美观。
Abstract:
Objective Study on the aesthetic effect of fi ne suture combined with local flap repair in the treatment of patients with periorbital tissue injury and its effect on scar condition after wound healing. Methods A total of 89 patients with facial trauma combined with orbital fracture admitted to our hospital were selected from June 2018 to June 2021.46 cases were divided into study group and 43 cases were control group,the control group was repaired with traditional suture technique,the study group was repaired with fi ne suture combined with local flap repair.The preoperative and postoperative clinical curative effect,degree of scarring (Vancouver scar scale,VSS), Scar beauty (patient and observer scar assessment scales ,POSAS) of the two groups were compared. Results The eff ective rate of the study group(97.83%) was significantly higher than that of the control group(86.05%) after the intervention,the difference was statistically significant( P＜0.05).After treatment, VSS scores in both groups were lower than before treatment,the study group (2.08±0.62) decreased more significantly than the control group (3.66±0.53),the difference was statistically significant( P＜0.05), POSAS scores decreased in both groups after treatment,the study group (11.33±2.15) decreased more significantly than the control group (15.26±2.09),the difference was statistically significant( P＜0.05), The hardness of scar was reduced after treatment,the study group (0.63±0.18) decreased more significantly than the control group (0.71±0.16).After repair, there was a statistical difference ( P＜0.05) in the number of cases of complete eyelid repositioning in the study group (43/46) compared to the control group (30/43), no statistical difference (P＞0.05) in the number of cases of lid ectropion in the study group (2/46) compared to the control group (5/43), and no statistical difference (P＞0.05) in the number of cases of lid bulb separation in the study group (4/46) compared to the control group (8/43). Conclusion Fine suture combined with local flap repair in the repair of orbital fracture,can effectively reduce the degree of scar after healing,make the scar beautiful and the wound more beautiful after repair.
