摘要:

目的：探究拔牙矫治成年女性错牙合畸形对其侧貌的影响。方法：选择2014年10月-2018年5月北京市垂杨柳医院口腔科 就诊的80例成年女性错牙合畸形患者为研究对象，根据矫治方法不同分为拔牙组（49例）和非拔牙组（31例），回顾性收集患 者临床资料。拔牙组接受拔牙矫治，非拔牙组接受非拔牙矫治。观察比较两组治疗前后头影像学检测、软组织测量值、上气 道形态及舌骨位置。结果：治疗后，两组下颌骨长度较治疗前延长，拔牙组长于非拔牙组；PNS-UPW、SNA较治疗前升高，拔 牙组高于非拔牙组；SNB、ANB较治疗前降低，拔牙组低于非拔牙组，差异均有统计学意义（P＜0.05）。拔牙组上中切牙倾 角、下中切牙倾角、UL-LL较治疗前降低，非拔牙组上中切牙倾角、下中切牙倾角升高，拔牙组低于非拔牙组，差异均有统 计学意义（P＜0.05）；拔牙组Sn-UL、LL-Sm、SPP-SPPW较治疗前升高，拔牙组高于非拔牙组（P＜0.05）。治疗后，拔牙组 H-FH、H-MP较治疗前升高，非拔牙组H-FH较治疗前升高（P＜0.05），但组间比较差异均无统计学意义（P＞0.05）。结论： 相较非拔牙矫治，拔牙矫治可改善头影像测量指标及上气道形态，并可提高成年女性错牙合畸形患者侧貌软组织的协调程度。

Abstract:

Objective To explore the eff ect of tooth extraction on the profi le of adult female malocclusion. Methods Between October 2014 and May 2018, 80 adult female patients with malocclusion were divided into the extraction group (49 cases) and the non extraction group (31 cases) by the method of correction, and their clinical data were collected retrospectively. The extraction group received extraction treatment, and the non extraction group received non extraction treatment. The head imaging, soft tissue measurement, upper airway morphology and hyoid bone position were compared between the two groups. Results After treatment, the length of the mandible of the two groups was longer than before treatment, and the extraction group was longer than the non extraction group; PNS-UPW, SNA of the two groups increased compared with those before treatment, and those in the extraction group were higher than those in the non extraction group (P ＜0.05); SNB and ANB decreased compared with before treatment, and those in the tooth extraction group was lower than the non extraction group (P＜0.05), the upper central incisor angle, the lower central incisor inclination angle and UL-LL in the extraction group were lower than those before treatment, while the upper central incisor angle and the lower central incisor inclination angle in the non extraction group were higher than those in the non extraction group (P＜0.05); the Sn-UL, LLSm, SPP-SPPW in the extraction group were higher than those before treatment, those in the extraction group were higher than the non extraction group (P＜0.05).After treatment, the H-FH, H-MP in the extraction group were higher than those before treatment, and the H-FH in the non extraction group increased than before treatment(P＜0.05), but there was no signifi cant diff erence between the two groups (P＞0.05). Conclusion Compared with non extraction treatment, extraction treatment could improve the cephalometric parameters and upper airway morphology, and improve the coordination degree of soft tissue in the profi le of adult female patients with malocclusion .
