目的：对比同指指动脉逆行岛状皮瓣与指动脉背侧支岛状皮瓣修复指端缺损的临床治疗效果。方法：回顾性选取2015 年4月-2020年5月笔者医院收治的192例指端缺损患者临床资料，根据不同皮瓣修复术分为三组，其中行邻指皮瓣修复的56 例患者为对照组，行指动脉逆行岛状皮瓣修复的70例患者为逆行组，行指动脉背侧支岛状皮瓣修复的66例患者为背侧支组， 比较三组手术时间、住院时间、指关节活动度、两点辨别觉、皮瓣感觉功能、患指皮肤颜色、术后并发症及患指预后情况。 结果：三组手术时间、近侧指关节活动度及远侧指关节活动度比较，差异均无统计学意义（P＞0.05）；逆行组两点辨别觉 优于对照组和背侧支组（P＜0.05），逆行组与背侧支组住院时间短于对照组（P＜0.05）。术后1年，三组皮瓣感觉功能优 良率数值比较，逆行组＞背侧支组＞对照组，三组间及两两组间比较差异均有统计学意义（P＜0.05）。术后半年，三组患 指皮肤颜色优良率数值比较，逆行组＞背侧支组＞对照组，三组间及两两组间比较差异均有统计学意义（P＜0.05）。术后 半年内，逆行组术后并发症发生率低于对照组和背侧支组，差异均有统计学意义（P＜0.05）。术后1年，逆行组患指各指标 （皮瓣血运、外形美观情况及感觉功能状态）均优于对照组和背侧支组，差异均有统计学意义（P＜0.05）。结论：同指指 动脉逆行岛状皮瓣与指动脉背侧支岛状皮瓣均可修复指端缺损，但指动脉逆行岛状皮瓣修复其术后并发症发生率更低、外形 美观状态更好、皮瓣颜色恢复更佳、皮瓣感觉功能更优，值得临床推广。
Abstract:
Objective To investigate the clinical eff ects of retrograde island fl ap of the same fi nger artery and the digital artery dorsal branch island fl ap in repairing fi ngertip defects. Methods Clinical data of 192 patients with fi nger tip defect admitted to the author’s hospital from April 2015 to May 2020 were retrospectively selected and divided into three groups according to diff erent fl ap repair operations. Among them,56 patients with adjacent fi nger fl ap repair were the control group,and 70 patients with reverse digital artery island fl ap repair were the reverse group. Sixty-six patients who underwent dorsal branch of digital artery island flap were included in the dorsal branch group. The operation time,length of stay,range of motion of knuckle joint,two-point discrimination,sensory function of the fl ap,skin color of the fi nger,postoperative complications and prognosis of the fi nger were compared among the three groups. Results There were no signifi cant diff erences in operating time,proximal and distal phalangeal range of motion among the three groups (P ＞0.05). Two-point discrimination in the retrograde group was better than that in the control group and the dorsal rami group (P＜0.05),and the length of hospitalization in the retrograde group and the dorsal rami group was shorter than that in the control group (P＜0.05). One year after surgery,the superior and excellent rate of sensory function of the three groups was compared,the retrograde group ＞ the dorsal rami group ＞ the control group,and there were statistically signifi cant diff erences among the three groups and between the two groups (P ＜0.05). Half a year after surgery,the good and good rate of skin color of the aff ected fi nger in the three groups was compared,the retrograde group ＞ the dorsal rami group ＞ the control group,and there were statistically signifi cant diff erences among the three groups and between the two groups (P ＜0.05). Within half a year after surgery,the incidence of postoperative complications in the retrograde group was lower than that in the control group and the dorsal rami group,with statistical signifi cance (P ＜0.05). One year after surgery,all indexes of the aff ected fi nger in the retrograde group (skin fl ap blood circulation,appearance and sensory function) were better than those in the control group and the dorsal rami group,with statistical signifi cance (P ＜0.05). Conclusion Both the retrograde island fl ap of the same fi nger artery and the digital artery dorsal branch island fl ap can be used to repair fingertip defects. However,the former can achieve a lower incidence of postoperative complications,better appearance,color recovery and fl ap sensory function,worthy of clinical promotion.
