摘要:

目的：探究iRoot BP Plus对前牙根尖周病根管倒填充治疗患者局部肿痛情况、牙周炎症和美学效果的影响。方法： 按照随机数字表法将2020年1月-2022年2月山西省中西医结合医院口腔科收治的65例前牙根尖周病患者分为观察组（33例） 和对照组（32例）。对照组和观察组分别采用氢氧化钙糊剂和iRoot BP Plus进行根管倒充填。对比两组患者治疗后肿 痛情况，治疗前及治疗后6、12个月旧根尖周指数（Old-periapical index，O-PAI），龈沟液炎症因子水平及患者对 美学效果的满意度。结果：治疗后，观察组肿痛率为3.03%，低于对照组的21.88%（P＜0.05）；两组O-PAI的时间效 应、组间效应及交互效应比较差异有统计学意义（F=99.320、14.870、7.596，均P＜0.05）；治疗后两组患者O-PAI随 着时间延长而降低，且观察组低于对照组（P＜0.05）；治疗后，两组患者龈沟液中白细胞介素-6和白细胞介素-1β水平均 降低，且观察组低于对照组（P＜0.05）；治疗后，观察组患者对牙齿咬合度、咀嚼功能、牙齿色泽和整体美观性的满意度 评分均高于对照组（P＜0.05）。结论：前牙根尖周病患者采用iRoot BP Plus倒充填根管治疗，可有效降低患者牙龈红肿、 缓解牙周炎症并提高患者对美观性的满意度。

Abstract:

Objective The purpose of this study was to investigate the eff ect of iRoot BP Plus on local redness and swelling, periodontal inflammation and aesthetics of affected teeth in patients with anterior teeth periapical disease undergoing root canal therapy. Methods A total of 65 patients with anterior teeth periapical disease who were admitted to the Department of Stomatology at Shanxi Provincial Integrated Traditional Chinese Medicine and Western Medicine Hospital from January 2020 to February 2022 were divided into an observation group (33 cases) and a control group (32 cases) by random number table method. The control group and the observation group underwent filling with calcium hydroxide pastes and iRoot BP Plus, respectively. The condition of postoperative swelling and pain, old periapical index (O-PAI), levels of infl ammatory factors in gingival crevicular fl uid, and patient satisfaction with aesthetics of aff ected teeth were compared between the two groups. Results After treatment, the rate of swelling and pain in the observation group (3.03%) was lower than that in the control group (21.88%) (P＜0.05). There were statistically significant differences in O-PAI in terms of time effect, inter-group effect and interaction eff ect (F=99.320, 14.870, 7.596, all P＜0.05). After operation, O-PAI in the two groups decreased over time, and O-PAI in the observation group was lower than that in the control group (P＜0.05). After treatment, the levels of interleukin-6 and interleukin-1β in gingival crevicular fl uid of the two groups decreased, and levels in the observation group were lower than those in the control group (P＜0.05). After treatment, patients in the observation group were more satisfi ed with tooth occlusion, masticatory function, tooth color and overall appearance than those in the control group (P＜0.05). Conclusion Applying iRoot BP Plus fi lling in root canal therapy for patients with anterior teeth periapical disease can eff ectively reduce gingival redness and swelling, alleviate periodontal infl ammation, and improve the patients’ satisfaction with aesthetics of aff ected teeth.
