[摘要]目的：探究果酸换肤术联合五积散对痤疮患者皮肤屏障的修复和内分泌调节的作用。方法：选取2018年5月-2019年3
月于保山市人民医院皮肤科就诊的寒湿郁滞、脾胃湿热型痤疮患者120例，随机分为中药组、西药组、中西医联合组和空白
对照组，每组30例。中药组采用五积散免煎颗粒治疗，西医选用果酸治疗，联合组采取中西医结合治疗，空白对照组不予治
疗。比较四组的皮损积分、皮肤经皮水丢失（TEWL）、角质层含水量、pH值和证候积分。结果：治疗后，联合组的皮损积分
低于西药组、中药组及空白对照组，空白对照组的皮损积分高于西药组、中药组（P ＜0.05），治疗后较治疗前比较，除空
白对照组外，其余的皮损积分均降低（P ＜0.05）。治疗后，四组患者的鼻部TEWL无统计学差异（P ＞0.05）；但是四组的前
额、脸颊TEWL不同，空白对照组、西药组及中药组的前额、脸颊TEWL均低于联合组（P ＜0.05）。空白对照组的前额、脸颊
TEWL均低于西药组、中药组（P ＜0.05），联合组的前额、脸颊TEWL均较治疗前降低（P ＜0.05）；四组患者的鼻部角质层含
水量无统计学差异（P ＞0.05）；但空白对照组、西药组及中药组的前额、脸颊角质层含水量均低于联合组（P ＜0.05）。
治疗前后比较，联合组的前额、脸颊角质层含水量均较治疗前降低（P ＜0.05）。治疗前后，四组患者的pH值均在正常范围
内，且均无统计学差异（P ＞0.05）。治疗后，四组患者的证候积分两两不同，除空白对照组外，其余三组患者的证候积分
均降低（P ＜0.05）。结论：果酸联合五积散治疗寒湿郁滞、脾胃湿热型寻常性痤疮，能更好地修复患者皮肤屏障，改善患
者内分泌指标，可临床推广应用。
Abstract:
Abstract: Objective To explore the effect on skin barrier and endocrine regulation in the treatment of patients with acne
vulgaris using fructonic acid dermatoplasty with Wuji powder. Methods 120 patients with acne vulgaris who were treated in
the dermatology department of our hospital from May 2018 to March 2019 were randomly divided into four groups:the Chinese
medicine group was treated with Wujisan decoction-free granules, while the western medicine was treated with fructonic acid
and the combined group with the combined treatment, and the blank control group was not treated for 4 weeks. The Skin lesion
score, TEWL, cuticle water content, pH value and syndrome scores in the four groups were compared. Results After treatment,
the scores of skin lesions in the combined group were lower than those in the western medicine group, the traditional Chinese
medicine group and the blank control group. the scores of skin lesions in the blank control group were higher than those in the
western medicine group and the traditional Chinese medicine group (P ＜0.05). After treatment, the scores of other skin lesions
were lower than those before treatment except the blank control group (P ＜0.05). After treatment, there was no significant
difference in nasal TEWL between the four groups (P ＞0.05). However, the forehead and cheek TEWL in the four groups are
different. The forehead and cheek TEWL in the blank control group, western medicine group and traditional Chinese medicine
group were lower than those in the combined group (P ＜0.05). The TEWL of forehead and cheek in the blank control group
were lower than those in the western medicine group and the Chinese medicine group (P ＜0.05), while the TEWL of forehead
and cheek in combined group were lower than those before treatment (P ＜0.05). After treatment, there was no statistical
difference in nasal stratum corneum water content between the four groups (P ＞0.05). However, the water content of forehead
and cheek cuticle in blank control group, western medicine group and traditional Chinese medicine group was lower than
that in combined group (P ＜0.05). After treatment and before treatment, the water content of forehead and cheek cuticle in
the combined group was lower than before treatment (P ＜0.05). Before and after treatment, the pH values of the four groups
of patients were all within the normal range, and there was no statistical difference (P ＞0.05). After treatment, the syndrome
scores of the four groups of patients were different from each other. except for the blank control group, the syndrome scores of
the other three groups of patients were reduced (P ＜0.05). Conclusion Fructonic acid dermatoplastywith Wuji powder in the
treatment of patient with ane vulgaris can repair the skin barrier better, improve the patient’s endocrine index, value of clinical
promotion and reference.
