摘要:
[摘要]目的:探究远红外频谱照射联合水凝胶敷料治疗烧伤创面的效果及对血清促红细胞生成素(Erythropoietin,
EPO)、转化生长因子-α(Transforming growth factor-α,TGF-α)表达的影响。方法:应用前瞻性随机对照研究方
法选取2017年10月-2019年12月笔者医院烧伤患者116例,以随机数字表法分为研究组(n =58)、对照组(n =58)。常规
处理基础上,给予对照组水凝胶敷料治疗,给予研究组远红外频谱照射联合水凝胶敷料治疗,两组均治疗3周。对比两组
疗效、创面愈合时间及创面感染率,比较治疗前、治疗3周后创面疼痛程度(VAS评分)、瘢痕程度(VSS评分)、血清炎
症因子指标[肿瘤坏死因子-α(TNF-α)、C反应蛋白(CRP)、白细胞介素-6(IL-6)]、致痛因子指标[前列腺素E2
(PGE2)、神经肽(NPY)、5-羟色胺(5-HT)]、EPO及TGF-α水平。结果:治疗3周后,研究组总有效率(93.10%)高于
对照组(77.59%),差异有统计学意义(P <0.05)。研究组创面愈合时间短于对照组,创面感染率(6.90%)低于对照组
(20.69%),差异有统计学意义(P <0.05)。研究组治疗3周后VAS评分低于对照组,色泽、血管分布、厚度、柔软度评分
低于对照组,差异有统计学意义(P <0.05)。研究组治疗3周后血清TNF-α、CRP、IL-6、PGE2、NPY、5-HT水平低于对照
组,差异有统计学意义(P <0.05)。研究组治疗3周后血清EPO、TGF-α水平高于对照组,差异有统计学意义(P <0.05)。
结论:远红外频谱照射联合水凝胶敷料治疗烧伤患者,能进一步提高血清EPO、TGF-α水平,抑制炎症因子、致痛因子表
达,从而有效降低疼痛程度,且能促进创面愈合,改善患者预后,疗效显著。
Abstract:
Abstract: Objective To explore the effect of far-infrared spectrum irradiation combined with hydrogel dressing on burn
wounds and its effect on the expression of serum erythropoietin (EPO), transforming growth factor-α (TGF-α). Methods A
prospective randomized controlled study was used to select 116 patients with burns in our hospital from October 2017 to
December 2019, and the patients were divided into the research group (n =58) and the control group (n =58) by a random number
table method. On the basis of conventional treatment, the control group was given hydrogel dressing treatment, and the research
group was given far infrared spectrum irradiation combined with hydrogel dressing treatment, and both groups were treated
for 3 weeks. Comparing the curative effect, wound healing time, wound infection rate, the degree of wound pain (VAS score),scar
degree (VSS score), serum infl ammatory factor indexes [tumor necrosis factor-α (TNF-α), C-reactive protein (CRP), interleukin-6
(IL -6)], indicators of pain factors [prostaglandin E2 (PGE2), neuropeptide (NPY), serotonin (5-HT)], EPO, TGF-α levels before
treatment and after 3 weeks of treatment of the two groups. Results After 3 weeks of treatment, the total effective rate of the
research group (93.10%) was higher than that of the control group (77.59%), the difference was statistically signifi cant (P <0.05).
The wound healing time of the research group was shorter than that of the control group, and the wound infection rate (6.90%)
in the research group was lower than that of the control group (20.69%, P <0.05). The VAS score of the research group after 3
weeks of treatment was lower than that of the control group (P <0.05). The scores of color, blood vessel distribution, thickness,
and softness of the research group after 3 weeks of treatment were lower than those of the control group (P <0.05). The level of
serum TNF-α, CRP, IL-6, PGE2, NPY, and 5-HT in the research group was lower than those in the control group after 3 weeks
of treatment (P <0.05). The level of serum EPO and TGF-α in the research group was higher than those in the control group
after 3 weeks of treatment (P <0.05). Conclusion Far infrared spectrum irradiation combined with hydrogel dressing for burn
patients can further increase serum EPO and TGF-α levels and inhibit the expression of infl ammatory factors and pain-causing
factors, thereby, it can effectively reduce the degree of pain, and can promote wound healing, improve the prognosis of patients,
and has a signifi cant effect.
