摘要:

目的：分析烧伤植皮联合综合性康复护理干预对手部烧伤患者的治疗效果。方法：2017年1月-2020年2月于笔者医院烧伤科收治的122例手部烧伤患者随机分为对照组和观察组，各61例。两组均采用烧伤植皮手术进行治疗，对照组术后住院期间采用常规护理干预，观察组在对照组的基础上采用综合性康复护理干预，术后随访3个月。比较两组术前和术后3个月手部运动功能、简明烧伤健康量表（BSHA-A）评分及瘢痕情况，统计两组术后住院期间并发症发生情况。结果：与术前比较，术后3个月两组腕关节背伸活动度、手指关节总活动度（TAM）评分、手功能评分及躯体功能、一般健康状况、社会关系、心理功能评分均升高，且观察组高于对照组，差异有统计学意义（P＜0.05）。与术前比较，术后3个月两组瘢痕色泽、血管分布、厚度、柔软度评分均降低，且观察组低于对照组，差异有统计学意义（P＜0.05）。术后住院期间，观察组并发症总发生率与对照组比较，差异无统计学意义（P＞0.05）。结论：在烧伤植皮术后采用综合性康复护理对手部烧伤患者进行干预，可有效改善患者手部运动功能，提高患者健康状况，同时改善瘢痕色泽、提高柔软度，降低瘢痕厚度及血管分布情况，同时并发症少。

Abstract:

Objective To analyze the therapeutic effect of burn skin grafting combined with comprehensive rehabilitation nursing intervention on hand burn patients. Methods From January 2017 to February 2020, 122 cases of hand burn patients in our hospital were taken as research objects, and they were randomly divided into the control group and the observation group by random number table method, with 61 cases in each group. Both groups were treated with skin grafting, while the control group received routine nursing intervention during hospitalization after operation, while the observation group received comprehensive rehabilitation nursing intervention on the basis of the control group. Both groups were followed up for 3 months after operation. Hand motor function, rief scale of burn health-A (BSHA-A) scale score and scar status were compared between the two groups before and 3 months after operation, and the complications during postoperative hospitalization were counted. Results Compared with pre-operation, the range of motion of wrist dorsiflexion, the scores of total range of motion (TAM), hand function score, physical function, general health condition, social relations and psychological function scores of the two groups increased 3 months after operation, and the control group was lower than the observation group (P＜0.05). Compared with pre-operation, the scores of color, blood vessel distribution, thickness and softness scar in the two groups decreased 3 months after operation, and the scores in the observation group were lower than those in the control group (P＜0.05). During postoperative hospitalization, the total incidence of complications in the observation group was not statistically significant compared with that in the control group (P＞0.05). Conclusion Comprehensive rehabilitation nursing intervention for hand burn patients after skin grafting could effectively improve the hand movement function and health status of patients, improve scar color and softness, reduce scar thickness and blood vessel distribution, and there was fewer complications.
