摘要:
目的：研究沟通表里和解祛邪法治疗银屑病的临床疗效及对皮肤症状的改善作用。方法：选取2019年12月-2020年9月笔者医院诊治的158例银屑病患者作为研究对象，按照随机数字表法均分为两组，对照组79例采用西药阿维A胶囊及扑尔敏、维生素E对症治疗，观察组79例在对照组基础上增加沟通表里和解祛邪法治疗。比较两组患者的临床疗效，皮肤症状变化情况，炎性因子水平，白细胞p38MAPK活化水平，以及不良反应发生情况。结果：观察组总有效率为97.5%，略高于对照组的93.7%，组间差异无统计学意义（P＞0.05）；但观察组愈显率为84.8%，显著高于对照组的70.9%，组间差异有统计学意义（P＜0.05）。治疗后两组患者皮肤症状各项评分均较治疗前显著下降（P＜0.05），并且治疗后观察组红斑、鳞屑评分显著低于对照组（P＜0.05）。两组患者治疗后血清炎症因子水平均较治疗前显著降低（P＜0.05），并且观察组低于对照组（P＜0.05）。治疗后，两组白细胞p38MAPK阳性细胞百分比评分、染色强度评分均显著低于治疗前（P＜0.05），且观察组低于对照组（P＜0.05）。观察组、对照组不良反应发生率分别为5.1%、6.3%，差异无统计学意义（P＞0.05）。结论：沟通表里和解祛邪法治疗银屑病的临床疗效满意，能够抑制炎性反应，显著改善红斑、角质层厚度、鳞屑症状，有利于控制疾病进展。
Abstract:
Objective To study the clinical efficacy of the treatment of psoriasis and the improvement effect on the skin symptoms. Methods A total of 158 patients with psoriasis diagnosed and treated in the author’s hospital from December 2019 to September 2020 were selected as the research objects. They were divided into two groups according to the random number table method. The control group 79 cases were treated with western medicine Acitretin capsules and chlorpheniramine. Vitamin E was used for symptomatic treatment. The 79 cases in the observation group were treated with the method of communication between the outside and the inside and the method of eliminating evils on the basis of the control group. Compare the clinical efficacy, the changes of skin symptoms, the level of inflammatory factors, the activation of p38MAPK in leukocytes and occurrence of adverse reactions were compared between the two groups. Results The total effective rate of the observation group was 97.5%, slightly higher than the 93.7% of the control group, the difference between the groups was not statistically significant (P ＞0.05), but the cure rate of the observation group was 84.8%, which was significantly higher than the control group’s 70.9%. The difference between the groups was statistically significant (P＜0.05). After treatment, the scores of TCM skin symptoms of the two groups of patients were significantly lower than those before treatment (P＜0.05), and the scores of erythema and scales in the observation group were significantly lower than those in the control group (P＜0.05). After treatment, the levels of serum inflammatory factors in the two groups were significantly lower than before treatment (P＜0.05), and the observation group was lower than the control group (P＜0.05). After treatment, the white blood cell p38MAPK positive cell percentage score and staining intensity score of the two groups were significantly lower than before treatment (P＜0.05), and the observation group was lower than the control group (P＜0.05). The incidence of adverse reactions in the observation group and the control group were 5.1% and 6.3%, respectively, and the difference was not statistically significant (P＞0.05). Conclusion The clinical curative effect of the method of reconciling exterior and interior and eliminating evil in the treatment of psoriasis is satisfactory, which can inhibit the inflammatory reaction, significantly improve the symptoms of erythema, stratum corneum thickness and scales, and is conducive to the control of disease progression
