摘要:
目的：探讨视频健康教育结合音乐疗法对下颌阻生牙拔除患者预后的影响。方法：选取2018年1月-2019年12月在笔者医院进行下颌阻生牙拔除的患者60例，随机数值表法分为观察组和对照组各30例。两组均由同一组医生实施常规下颌阻生牙拔除术，对照组在拔牙期间行常规性护理，观察组在对照组的基础上实施视频健康教育干预结合音乐疗法。记录两组手术时间、术后1 d时的面部肿胀程度及术后并发症情况；测量两组干预前、术中（拔牙开始后10 min）的心率、收缩压（Systolic blood pressure，SBP）、舒张压（Diastolic blood pressure，DBP）；于干预前后，采用改良Corah’S牙科焦虑量表（Modified dental anxiety scale，MDAS）和牙科畏惧调查量表（Dental fear scale，DFS）中文版评估患者牙科焦虑、畏惧程度。结果：两组术后1 d面部肿胀程度比较差异无统计学意义（P＞0.05）。观察组手术时间明显短于对照组，差异有统计学意义（P＜0.05）。两组术中心率、SBP、DBP指标均较本组干预前明显上升，但观察组术中心率、SBP、DBP指标明显低于对照组，差异有统计学意义（P＜0.05）。干预后MDAS、SDS评分均较本组术前明显下降，且观察组干预后MDAS、SDS评分明显低于对照组，差异有统计学意义（P＜0.05）。观察组术后并发症总发生率为13.33%，低于对照组的23.33%，但组间比较差异无统计学意义（P＞0.05）。结论：视频健康教育结合音乐疗法能有效改善下颌阻生牙拔除患者的牙科焦虑、畏惧情绪，稳定术中生理指标，缩短手术时间。
Abstract:
Objective To explore the effects of video health education combined with music therapy on the prognosis of patients with mandibular impacted teeth extraction. Methods A total of 60 patients who underwent mandibular impacted teeth extraction in the hospital from January 2018 to December 2019 were enrolled. They were divided into the observation group and the control group by random number table method, 30 cases in each group. Both groups underwent mandibular impacted teeth extraction by the same group of doctors. The control group was given routine nursing during extraction, while observation group was given video health education combined with music therapy on basis of control group. The operation time, severity of facial swelling at 1 d after surgery and postoperative complications in both groups were recorded. The heart rate, SBP and DBP before intervention and during surgery (at 10 min after the start of extraction) in both groups were measured. Before and after intervention, dental anxiety and fear were evaluated by MDAS and DFS. Results There was no signifi cant diff erence in the degree of facial swelling between the two groups 1 day after operation (P＞0.05). Operation time in the observation group was signifi cantly shorter than that in the control group (P＜0.05). Intraoperative heart rate, SBP and DBP were signifi cantly higher than those before intervention in both groups (P＜0.05), which in the observation group were signifi cantly lower than the control group (P＜0.05). MDAS and SDS scores after intervention were signifi cantly lower than those before surgery in both groups (P＜0.05), in the observation group which were signifi cantly lower than control group (P＜0.05). The total incidence of postoperative complications in the observation group was lower than that in the control group (13.33% vs 23.33%), but there was no signifi cant diff erence between the groups (P＞0.05). Conclusion Video health education combined with music therapy can eff ectively improve the dental anxiety and fear of patients undergoing impacted wisdom teeth extraction, stabilize intraoperative physiological indexes, and shorten operation time
