摘要:

目的：探究积极心理学理论指导下全程护理干预在面部烧伤后瘢痕整形患者中的应用效果。方法：选取笔者医院2018 年4月-2021年4月收治的面部烧伤后瘢痕整形患者100例，随机数表法分为观察组和对照组，各50例。对照组采用常规护理干 预，观察组给予积极心理学理论指导下全程护理干预。观察两组患者的创面情况、情绪状态、生活质量和应对方式。结果： 观察组愈合时间、色素沉着率和瘢痕增生率低于对照组，差异无统计学意义（P＞0.05）。干预后两组汉密尔顿焦虑量表 （Hamilton anxiety scale，HAMA）评分和汉密尔顿抑郁量表（Hamilton depression scale，HAMD）评分均降低，且观察 组低于对照组，差异有统计学意义（P＜0.05）。观察组躯体功能、社会功能、心理功能、物质功能评分均高于对照组，差 异有统计学意义（P＜0.05）。干预后，两组面对评分均升高，且观察组高于对照组；两组回避、屈服评分均降低，且观察 组低于对照组，差异均有统计学意义（P＜0.05）。结论：积极心理学理论指导下全程护理对面部烧伤后瘢痕整形患者的干 预效果好，能够有效减少患者色素沉着率、瘢痕增生率，改善其情绪状态和生活质量，加快患者恢复进程。

Abstract:

Objective To explore the application effect of whole-course nursing intervention under the guidance of positive psychology theory in patients with scar plastic after facial burn. Methods 100 patients with scar plastic after facial burn who were treated in the author’s hospital from April 2018 to April 2021 were selected and randomly divided into the observation group and the control group, with 50 cases each. The control group received routine nursing intervention, while the observation group was given the whole-course nursing intervention under the guidance of positive psychology theory. The wound condition, emotional state, quality of life and coping style of the two groups were observed. Results The healing time, pigmentation rate and scar hyperplasia rate of the observation group were lower than those of the control group, but the diff erences were not statistically signifi cant (P＞0.05). After the intervention, the HAMA score and HAMD score of the two groups were decreased, and the observation group was lower than the control group, and the differences were statistically significant (P＜0.05). The scores of physical function, social function, psychological function and material function in the observation group were higher than those in the control group (P＜0.05). After the intervention, the face scores of the two groups increased, and the observation group was higher than the control group (P＜0.05). The avoidance and yield scores of the two groups decreased, and the observation group were lower than the control group (P＜0.05). Conclusion Whole-course nursing intervention under the guidance of positive psychology theory can eff ectively reduce pigmentation rate and scar hyperplasia rate, improve emotional state and quality of life, and accelerate the recovery process of patients undergoing scar plastic surgery after facial burn, with good eff ect.
