摘要:

目的：探讨重组人生长激素对重度烧伤患者的临床疗效及对炎症反应和免疫功能的影响。方法：选取2019年11 月-2020年7月收治的62例重度烧伤患者为研究对象，随机分为观察组和对照组，各31例。对照组患者皮下注射0.9%氯化钠注 射液1 ml，1次/天；观察组患者皮下注射重组人生长激素0.3 IU/kg，1次/天。两组患者均治疗20 d。记录两组患者创面愈合 时间、住院时间；评估患者治疗满意度；治疗前后，检测患者血清内毒素含量、血清炎性细胞因子水平、外周血T淋巴细 胞占比及血清血浆蛋白水平。结果：观察组患者创面愈合时间及住院时间短于对照组，患者满意率高于对照组。治疗前， 两组患者血清内毒素含量、血清炎性细胞因子水平、外周血T淋巴细胞占比及血清血浆蛋白水平组间比较，差异无统计学 意义（P＞0.05）。治疗14 d、21 d后，观察组患者血清内毒素含量、血清炎性因子[肿瘤坏死因子-α（TNF-α）、白细胞 介素-1β（IL-1β）、白细胞介素-6（IL-6）、白细胞介素-8（IL-8）、白细胞介素-12（IL-12）]水平均低于对照组，差 异均有统计学意义（P＜0.05）。治疗后，观察组患者CD3＋、CD8＋细胞占比均高于对照组（P＜0.05），CD4＋细胞占 比、CD4＋/CD8＋均低于对照组（P＜0.05）。治疗后，观察组患者血清血红蛋白、前白蛋白、血浆总蛋白水平均高于对 照组（P＜0.05）。结论：使用重组人生长激素治疗重度烧伤患者，能够促进患者创面恢复，降低患者血清内毒素及炎性因 子水平，调节患者细胞免疫功能及蛋白质代谢。

Abstract:

Objective To investigate the clinical effi cacy of recombinant human growth hormone in patients with severe burns and its eff ects on infl ammatory response and immune function. Methods Sixty-two patients with severe burn admitted from November 2019 to July 2020 were selected as the study objects,and randomly divided into observation group and control group,with 31 cases in each group. Control group was subcutaneously injected 0.9% sodium chloride injection 1 ml,once a day; Observation group was injected subcutaneously with recombinant human growth hormone 0.3 IU/kg,once a day. Both groups were treated for 20 days. Wound healing time and hospital stay of the two groups were recorded. Evaluate patient satisfaction with treatment; Before and after treatment,serum endotoxin content,serum inflammatory cytokine level,peripheral blood T lymphocyte proportion and serum plasma protein level were detected. Results The wound healing time and hospital stay in the observation group were shorter than those in the control group,and the satisfaction rate of patients was higher than that in the control group. Before treatment,there were no signifi cant diff erences in serum endotoxin content,serum infl ammatory cytokine level,peripheral blood T lymphocyte proportion and serum plasma protein level between 2 groups (P＞0.05). After 14 and 21 days of treatment,serum endotoxin content and serum infl ammatory factors [tumor necrosis factor-α (TNF-α),interleukin-1β (IL1β),interleukin-6 (IL-6),interleukin-8 (IL-8),interleukin-12 (IL-12)] in observation group were lower than those in control group. The diff erences were statistically signifi cant (P＜0.05). After treatment,the percentages of CD3+ and CD8+ cells in observation group were higher than those in control group (P＜0.05),while the percentages of CD4+ cells and CD4+ /CD8+ cells in observation group were lower than those in control group (P＜0.05). After treatment,the levels of serum hemoglobin,prealbumin and plasma total protein in observation group were higher than those in control group (P＜0.05). Conclusion Recombinant human growth hormone in the treatment of severe burn patients can promote wound recovery,reduce serum levels of endotoxin and infl ammatory factors,and regulate cellular immune function and protein metabolism.
