摘要:
目的：探讨眼轮匝肌瓣联合眶隔脂肪重置治疗上睑凹陷的临床效果。方法：笔者科室对收治的30例上睑凹陷就医者， 采用常规重睑成形的手术切口，但将重睑成形术中本应去除的组织（眼轮匝肌瓣）作为充填物，使其反折向上或自然回缩， 同时释放眶隔脂肪填充至凹陷区域，合并轻中度上睑下垂者采用联合腱膜缩短术，最后以常规切开重睑成形法缝合手术切 口。结果：术后3、6个月，总有效率均达96%（29/30），其中1例严重上睑凹陷就医者，因眼轮匝肌菲薄，睁眼时凹陷改善 效果不太理想，后期行颗粒脂肪填充，最终达到满意效果。结论：对于上睑凹陷的就医者，眼轮匝肌瓣充填和眶隔脂肪重置 能够增加上睑凹陷区域的组织容量，一期进行上睑下垂和内眦赘皮的矫正，眼部外形美观自然，术后无明显并发症，优于常 规上睑凹陷的解决方案，值得临床推广应用。
Abstract:
Objective To investigate the clinical eff ect of orbicularis muscle fl ap combined with septum orbitale fat reset in the treatment of upper eyelid depression. Methods In 30 cases of upper eyelid depression,a conventional blepharoplasty incision was made and the muscle (orbicularis oculi muscle fl ap) that would have been removed by conventional blepharoplasty was used as a fi ller to fi ll the sunken area by folding back and moving up or by natural retraction,while the septal fat was released to fi ll the sunken area of the upper eyelid. In those with combined mild to moderate ptosis,upper levator muscle combined with aponeurosis shortening was performed and the incision was fi nally closed with conventional incisional blepharoplasty sutures. Results At 3 and 6 months after operation,the total eff ective rate was 96% (29/30),including one patient with severe upper eyelid depression who had a thin orbicularis muscle and less satisfactory improvement of the depression when the eyes were open,which was later fi lled with fat particles to achieve satisfactory results. Conclusion In patients with upper eyelid depression,orbicularis muscle fl ap combined with septum orbitale fat reset can increase the tissue volume of the upper eyelid depression area,and correct both upper eyelid ptosis and epicanthus in one stage,with beautiful and natural appearance and no obvious postoperative complications,which is superior to the conventional solution of upper eyelid depression and worthy of clinical promotion and use.
