摘要:
目的:观察计算机辅助设计与制造(Computer aided design and manufacturing,CAD/CAM)种植导板技术用于上 颌牙缺失种植修复的美学效果。方法:收集笔者医院2016年6月-2020年6月间收治的71例上颌牙缺失患者资料,其中35例 采用常规牙种植修复治疗为对照组,36例采用CAD/CAM种植导板技术辅助种植为观察组,比较两组植入效果、植入体空间 位置偏离程度、牙周指标[牙齿松动度(Tooth movement,TM)、龈沟出血指数(Sulcus bleeding index,SBI)、菌斑指 数(Plaque index,PLI)、软垢指数(Debris index,DI)]、语言功能、咀嚼效率及美观度[红色复合美学指数(Pink esthetic score,PES)]。结果:观察组PIS分级优于对照组(P<0.05);两组植入体顶部偏差、底部偏差、角度偏差及深 度偏差均显著低于对照组(P<0.05);手术3个月后,两组TM、SBI、PLI、DI评分均显著降低(P<0.05),语言功能、咀 嚼效率均显著提高(P<0.05),且术后上述指标组间对比差异有统计学意义(P<0.05);观察组PES各指标评分均显著高 于对照组(P<0.05)。结论:上颌牙缺失患者采用CAD/CAM种植导板技术治疗,可显著提高种植精确度,患者术后语言、咀 嚼功能及美学效果较好,值得推荐。 [
Abstract:
Objective To observe the cosmetic effect of computer aided design and manufacturing (CAD/CAM) implanted guiding plate technology on implant restoration of maxillary tooth loss. Methods The data of 71 patients with maxillary tooth loss treated in the author’s hospital between June 2016 and June 2020 were collected. Among the patients, 35 cases treated with conventional dental implant restoration were selected as control group, and 36 cases assisted by CAD/CAM implanted guiding plate technology were enrolled as observation group. The implant eff ect, deviation degree of implant spatial position, periodontal indicators [tooth movement (TM), sulcus bleeding index (SBI), plaque index (PLI), debris index (DI)], language function, chewing effi ciency and aesthetics[Pink esthetic score (PES)] were compared between the two groups. Results The PIS classifi cation of observation group was better than that of control group (P <0.05). The top deviation, bottom deviation, angle deviation and depth deviation of implant in observation group were signifi cantly lower than those in control group (P <0.05). After 3 month of surgery, the scores of TM, SBI, PLI and DI in the two groups were signifi cantly reduced (P <0.05), and the language function and chewing effi ciency were signifi cantly improved (P <0.05), and there were signifi cant diff erences in the above-mentioned indicators between the groups after surgery (P <0.05). The scores of PES indicators in observation group were signifi cantly higher than those in control group (P <0.05). Conclusion The application of CAD/CAM implanted guiding plate technology can signifi cantly improve the accuracy of implant in the treatment of maxillary tooth loss, and it has good postoperative language, chewing function and cosmetic eff ect, thus it is worth recommending.
