摘要:
目的:分析正畸固定矫治中影响牙龈指数(Gingival index,GI)及牙釉质脱矿的危险因素。方法:选取2019年1 月-2021年1月于笔者医院行正畸固定矫治的180例患者为研究对象。收集患者资料,采用GI指数评估患者牙龈健康情况,采 用多元线性回归分析影响患者GI指数水平升高的独立危险因素;根据牙釉质脱矿检查结果分为脱矿组(n=105)和无脱矿组 (n=75),采用Logistic回归分析导致患者牙釉质脱矿的独立危险因素。结果:患者GI指数在年龄、性别、刷牙频率、刷牙 时间、是否使用牙线、睡前饮用含糖牛奶、含糖饮料、碳酸饮料及矫正器装置不良差异均有统计学意义(P<0.05);多元 线性回归分析结果显示,刷牙时间、睡前饮用含糖牛奶、含糖饮料及碳酸饮料及矫正器装置不良是影响正畸固定矫治患者 GI 指数水平的独立危险因素(均P<0.05)。单因素结果显示,两组患者年龄、刷牙频率、刷牙时间、是否改变口腔不良习 惯、碳酸饮料摄入频率、甜品摄入频率及正畸矫治时间差异均有统计学意义(P<0.05)。多因素分析结果显示,年龄、是 否改变口腔不良习惯、碳酸饮料摄入频率、甜品摄入频率及正畸矫治时间是影响正畸固定矫治患者牙釉质脱矿的独立危险因 素(均P<0.05)。结论:影响正畸固定矫治患者GI指数及牙釉质脱矿的危险因素较多,保持正确的口腔清洁习惯、控制含 糖食物及碳酸饮料的摄入、正确放置正畸固定器是预防患者GI指数升高及牙釉质脱矿的重要措施。
Abstract:
Objective To analyze the risk factors of gingival index (GI) and enamel demineralization in orthodontic treatment, and to construct a predictive nursing strategy. Methods A total of 180 patients who underwent orthodontic treatment in author’s hospital from January 2019 to January 2021 were selected as the research subjects. The data of patients were collected, GI index was used to evaluate the gingival health of patients, and multiple linear regression was used to analyze the independent risk factors aff ecting the level of GI index. In the mining group (n=75), Logistic regression was used to analyze the independent risk factors of enamel demineralization, and a predictive nursing strategy was further constructed. Results There were signifi cant diff erences in the GI index of patients in terms of age, gender, brushing frequency, brushing time, whether to use dental fl oss, drinking sugar-sweetened milk, sugar-sweetened beverages and carbonated beverages before going to bed, and poor orthodontic devices (P<0.05). The results of multiple linear regression analysis showed that brushing time, drinking sugar-sweetened milk, sugar-sweetened beverages and carbonated beverages before going to bed, and poor orthodontic devices were independent risk factors for the GI index level of orthodontic patients (all P<0.05). Univariate results showed that there were statistically significant differences in age, brushing frequency, brushing time, whether to change bad oral habits, carbonated beverage intake frequency, dessert intake frequency and orthodontic treatment time between the two groups (P<0.05). The results of multivariate analysis showed that age, whether to change bad oral habits, the frequency of carbonated beverage intake, the frequency of dessert intake and the time of orthodontic treatment were independent risk factors for enamel demineralization in patients with orthodontic treatment (all P<0.05). Conclusion There are many risk factors aff ecting GI index and enamel demineralization in patients with orthodontic fi xation. Maintaining correct oral hygiene habits, controlling the intake of sugary foods and carbonated beverages, and correct placement of orthodontic fi xtures can prevent patients from increasing GI index. and important measures of tooth enamel demineralization.
