摘要:

目的：探讨引导性组织再生术（Guided tissue regeneration，GTR）联合正畸治疗在牙周组织缺损伴前牙移位中的 应用及对牙周功能的影响。方法：选取笔者医院2018年1月-2021年9月收治的牙周组织缺损伴前牙移位患者92例，按照随机 数字表法分为研究组和对照组，每组46例，对照组给予GTR治疗，研究组在对照组的基础上联合正畸治疗。比较两组患者的 出血指数（Bleeding index，BI）、探诊出血（Probing bleeding，BOP）、菌斑指数（Plaque index，PLI）、牙周探诊深 度（Probing depth，PD）、附着丧失水平（Attachment loss，AL）、咀嚼功能、美观满意度、不良反应发生率以及临床疗 效。结果：治疗前，两组患者BI、BOP、PLI、PD、AL比较，差异无统计学意义（P＞0.05）；治疗后，研究组患者BI、BOP、 PLI、PD、AL及不良反应发生率均低于对照组，咀嚼效率、咬合能力、美观、口腔清洁、牙龈健康、整体满意度及治疗总有 效率均高于对照组（P＜0.05）。结论：GTR联合口腔正畸治疗牙周组织缺损伴前牙移位患者能够改善牙周功能和咀嚼功能， 提高患者美观度，降低不良反应发生率，临床疗效显著。

Abstract:

Objective To investigate the application of guided tissue regeneration (GTR) combined with orthodontic treatment in periodontal tissue defect with anterior tooth displacement and its eff ect on periodontal function. Methods 92 patients with periodontal tissue defect and anterior tooth displacement treated in author’s hospital from January 2018 to September 2021 were selected. Patients were randomly divided into study group and control group, with 46 cases in each group. The control group was treated with GTR, and the study group was treated with orthodontic treatment on the basis of the control group. The bleeding index (BI), probing bleeding (BOP), plaque index (PLI), periodontal probing depth (PD), attachment loss level (AL), masticatory function, aesthetic satisfaction, incidence of adverse reactions and clinical effi cacy were compared between the two groups. Results Before treatment, there was no signifi cant diff erence in BI, BOP, PLI, PD and Al between the two groups (P＞0.05). After treatment, BI, BOP, PLI, PD, AL and adverse reactions in the study group were lower than those in the control group, and the masticatory effi ciency, occlusal ability, aesthetics, oral hygiene, gum health, overall satisfaction, and total eff ective rate of treatment were all higher than those in the control group (P＜0.05). Conclusion GTR combined with orthodontics in the treatment of patients with periodontal tissue defect and anterior tooth displacement can improve periodontal function and masticatory function, improve patient aesthetics, reduce the incidence of adverse reactions, and the clinical eff ect is signifi cant.
