摘要:

目的：探讨基于态度-定义-开放思维-计划-实施（Attitude,definition,open mind,planning,try itout,ADOPT） 问题解决模式的个体化护理在手部软组织缺损皮瓣移植修复手术患者围手术期中的应用效果。方法：选取笔者医院2020年8 月-2022年1月收治的62例行手部软组织缺损皮瓣移植修复手术的患者为研究对象，根据围手术期护理干预方法分为观察组 （n=31例）与对照组（n=31例）。观察组患者在常规护理基础上实施基于ADOPT问题解决模式的个体化护理干预；对照组仅 实施常规护理干预。比较两组患者手部功能恢复情况、住院时间、心理状态变化情况、并发症发生情况及住院护理服务满意 度。结果：干预后，观察组手功能评估问卷（Michigan hand outcomes questionnaire,MHQ）评分高于对照组(P＜0.05)， 焦虑自评量表（Self-rating anxiety scale,SAS）、抑郁自评量表（Self-rating depression scale,SDS）评分均低于对 照组；观察组术后并发症发生率低于对照组，护理服务满意度高于对照组，但两组比较差异均无统计学意义(P＞0.05)；观 察组住院时间短于对照组(P＜0.05)。结论：基于ADOPT问题解决模式的个体化护理应用于手部软组织缺损皮瓣移植修复手术 患者围手术期中能有效促进患者手部功能恢复，改善不良心理情绪，缩短住院时间，提升住院服务满意度，值得临床推广。

Abstract:

Objective To explore the eff ect of ADOPT （Attitude,Defi nition,Open mind,Planning,Try it out ） problem solving model-based individualized nursing in the perioperative period of patients undergoing fl ap transplantation and restoration for hand soft tissue defects n. Methods 62 patients who underwent hand soft tissue defect fl ap transplantation and restoration in the author’s hospital from August 2020 to January 2022 were selected and divided into the observation group (n=31) and the control group (n=31) according to the perioperative nursing intervention method.The patients in the observation group were given ADOPT problem solving model-based individualized nursing intervention on the basis of conventional nursing,while the patients in the control group were only given conventional nursing intervention.The recovery of hand function,length of hospital stay,changes in psychological status,complications and satisfaction with inpatient nursing services were compared between the two groups. Results After intervention, the score of michigan hand outcomes questionnaire (MHQ) in the observation group was higher than that in the control group (P ＜0.05). The scores of self-rating anxiety scale (SAS) and self-rating depression scale (SDS) in the observation group were lower than those in the control group. The incidence of postoperative complications in the observation group was lower than that in the control group, and the satisfaction of nursing service was higher than that in the control group, but there was no statistically signifi cant diff erence between the two groups (P ＞0.05). Observation group in hospital was shorter than the control group (P＜0.05). Conclusion Individualized nursing based on ADOPT problem solving model can eff ectively promote the recovery of hand function,improve adverse psychological emotions,shorten the length of hospital stay and improve the satisfaction of inpatient services in the perioperative period of patients undergoing hand soft tissue defect fl ap transplantation and restoration.It is worthy of being widely popularized in the clinical application.
