摘要:

目的：探讨隐适美矫治器与Damon Q自锁托槽非拔牙矫治牙列拥挤患者的临床疗效。方法：选取2019年1月-2022年1 月在笔者医院完成非拔牙矫正治疗的100例牙列拥挤患者为研究对象，根据不同矫治方式分为隐适美组（使用隐适美矫治 器行正畸治疗）52例和Damon Q组（使用Damon Q自锁托槽矫治器行正畸治疗）48例。对比分析两组患者牙弓宽度指标（尖 牙间宽度、第一前磨牙间宽度、第一磨牙间宽度），牙周健康指标[牙龈指数（Gingival index,GI）、菌斑指数（Plaque index,PLI）、龈沟出血指数（Sulcus bleeding index,SBI）]，龈沟液炎症指标[白介素-1β（Interleukin-1β,IL1β）、肿瘤坏死因子-α（Tumor necrosis factor-α,TNF-α）]，以及口腔相关生活质量评分。结果：治疗结束后， 两组患者牙弓宽度均显著增加（P＜0.05），但两组比较差异无统计学意义（P＞0.05）。治疗后，两组患者GI、PLI、SBI、 IL-1β 、TNF-α指标均显著高于治疗前，且Damon Q组治疗1个月、2个月、3个月后各指标均高于隐适美组（P＜0.05）。治疗 期间，两组患者OHIP-14评分均高于治疗前，且Damon Q组治疗3个月、6个月后各指标评分均高于隐适美组（P＜0.05）；但 治疗结束后，两组患者OHIP-14评分均低于治疗前（P＜0.05）。结论：隐适美矫治器和Damon Q自锁托槽非拔牙矫治牙列拥 挤均有良好效果，可改善牙弓宽度，对患者牙齿口腔卫生影响较小，但隐适美矫治器更易保证牙周健康，控制牙周疾病的发 生，值得临床推广应用。

Abstract:

Objective To explore the clinical effect of non-extraction orthodontic correction with Invisalign appliance and Damon Q self-ligating bracket in patients with crowded dentition. Methods A total of 100 patients with crowded dentition undergoing non-extraction orthodontic correction in the hospital were enrolled between January 2019 and January 2022. According to diff erent orthodontic methods, they were divided into Invisalign group (orthodontic treatment with Invisalign appliance, 52 cases) and Damon Q group (orthodontic treatment with Damon Q self-ligating bracket, 48 cases). The indexes of dental arch width ( intercanine width, intercanine width of the fi rst premolar and molar), periodontal health [gingival index (GI), plaque index (PLI), sulcus bleeding index (SBI)], infl ammatory indexes in gingival crevicular fl uid [interleukin-1β (IL-1β), tumor necrosis factor-α (TNF-α)] and scores of oral health impactprofi le-14 (OHIP-14) in the two groups were compared. Results After treatment, width of dental arch was signifi cantly increased in both groups (P＜0.05), but diff erence between the two groups was not statistically signifi cant (P ＞0.05). After treatment, GI, PLI, SBI, IL-1β and TNF-α in both groups were signifi cantly increased, which were higher in Damon Q group than Invisalign group after 1, 2 and 3 months of treatment (P ＜0.05). During the treatment, OHIP-14 scores in both groups were increased, which were higher in Damon Q group than Invisalign group after 3 and 6 months of treatment (P＜0.05). After treatment, OHIP-14 scores in both groups were decreased (P＜0.05). Conclusion The curative eff ect of non-extraction orthodontic correction with both Invisalign appliance and Damon Q self-ligating bracket is good on crowded dentition, which can improve width of dental arch, with few eff ects on oral hygiene. However, Invisalign appliance is easier to ensure periodontal health and prevent the occurrence of periodontal diseases.
