摘要:
目的:探究女性型脱发的血清维生素D水平以及与雄激素的相关性,为女性型脱发的发病机制及治疗选择提供新的线 索。方法:分析皮肤科门诊就诊的70例女性型脱发患者与体检中心招募的65例健康人的血清维生素D,分析其与严重程度、 人口学特征、生活习惯以及雄激素的相关性。结果:女性型脱发患者的平均维生素D水平(13.59±5.82)ng/ml,健康人群 的平均维生素D水平为(15.94±6.89)ng/ml(P<0.05)。女性型脱发患者维生素D水平与性激素结合球蛋白呈正相关,与 游离睾酮呈负相关(P<0.05)。性激素结合球蛋白降低或游离睾酮升高的患者维生素D水平明显降低(P<0.05)。但暂未 发现女性型脱发患者的维生素D水平与严重程度、人口学特征、生活习惯的相关性。结论:女性型脱发较健康人群维生素D水 平明显降低,提示维生素D降低可能是女性型脱发的发病因素之一。性激素结合球蛋白降低、游离睾酮升高与维生素D水平降 低存在的相关性,可能是维生素D缺乏导致女性型脱发的机制之一,有待进一步研究验证。
Abstract:
Objective To explore the relationship between serum vitamin D levels and androgen levels in female pattern hair loss (FPHL), and to provide new clues for the pathogenesis and treatment of FPHL. Methods We analyzed serum vitamin D levels in 70 patients with female pattern alopecia and 65 healthy subjects recruited from hospital and physical examination center. We also analyzed the relationship between serum vitamin D levels and disease severity, demographic characteristics, lifestyle habits, and androgens. Results The mean levels of serum vitamin D signifi cantly decreased in FPHL, compared to those healthy controls[(13.59±5.82)ng/ml vs (15.94±6.89) ng/ml]. There was a positive correlation between vitamin D and sex hormone binding globulin (SHBG) (P<0.05), and a negative correlation between vitamin D and free testosterone (P<0.05). Serum vitamin D levels was signifi cantly lower in those patients with decrease SHBG or elevated free testosterone. However, no correlation was found between vitamin D level and severity, demographic characteristics, and lifestyle in FPHL. Conclusion The vitamin D level of FPHL is signifi cantly lower than that of healthy people, suggesting that the decrease of vitamin D may be one of the pathogenesis factors of FPHL. The correlation between SHBG, free testosterone and vitamin D may be the reason of the vitamin D defi ciency in FPHL, which needs more studies to verify. Serum vitamin D, SHBG and free testosterone should be monitored more actively in FPHL patients.
