[摘要]目的：观察应用前列地尔注射液对兔耳静脉瘀血皮瓣微循环的影响。方法：选取健康普通大耳白兔16只，随机分成生
理盐水对照组及前列地尔注射液治疗组，每组8只。麻醉后于左耳背制作大小6cm×3cm的静脉瘀血皮瓣模型。皮瓣成形术后
立刻于对照组右耳缘静脉缓慢注入生理盐水2ml，治疗组缓慢注入前列地尔注射2ml(1μg/1ml)，每天同一时间点注射一次至
实验结束，观察皮瓣外观及皮瓣存活率，分别于术后1、3、5、7d，各组分别随机取1只，切取皮瓣中点处皮瓣组织，HE染色
观察红细胞瘀积及炎症细胞浸润等情况，免疫组织化学染色观察皮瓣中VEGF及HIF-1α的表达情况。结果：治疗组皮瓣肿胀
情况、瘀血情况均明显较对照组轻。治疗组皮瓣成活率为（90.80±3.27）%明显高于对照组的（67.20±2.59）%，差异有统
计学意义（P ＜0.05）。HE染色结果示治疗组红细胞瘀积及炎症细胞浸润等情况明显较对照组轻；免疫组织化学结果示治疗
组皮瓣中VEGF的阳性表达量明显高于对照组，HIF-1α的阳性表达量明显低于对照组。结论：前列地尔注射液通过促进皮瓣
组织中VEGF的表达和降低皮瓣中HIF-1α的表达量，促进瘀血皮瓣中新生血管的生成速度及减轻瘀血皮瓣的缺血缺氧双重作
用改善瘀血皮瓣的微循环，提高静脉瘀血皮瓣的存活面积。
Abstract:
Abstract: Objective To observe the effect of alprostadil injection on microcirculation of rabbit auricular venous congestion
skin flaps. Methods Sixteen healthy rabbits were randomly divided into two groups: the control group(injecting saline group)
and the treatment group (alprostadil injection group), each of 8. After anesthesia, the model of venous congestion skin flap was
made on the back of the left ear with the size of 6cm×3cm. The treatment group was injected with alprostadil 2ml(1μg/1ml),
and the control group saline(2ml/flap) through the marginal vein of right ear slowly after the flap plasty, which were injected
at the same time every day until the end of the experiment, the appearance of the flaps and the survival rate of the flaps were
observed. On the 1d, 3d, 5d and 7d after operation, one of the flaps was randomly selected from each group. The tissue of
the flaps at the midpoint of the flaps was cut. After section, HE staining was used to observe erythrocyte deposition and inflammatory
cell infiltration. Immunohistochemical staining was used to observe the expression of vascular endothelial growth factor and HIF-1α in
the flaps. Results All of the flaps were swollen and congested after operation, the swelling and congestion of the flaps in the
treatment group were significantly lighter than those in the control group. The survival rate of flaps in the treatment group was
(90.80±3.27)%, which was significantly higher than that in the control group(67.20±2.59)%, the difference was statistically
significant(P ＜0.05). HE staining showed that erythrocyte deposition and inflammatory cell infiltration in the treatment group
were significantly lighter than those in the control group. Immunohistochemical results showed that the positive expression of
vascular endothelial growth factor in skin flaps of the treatment group was significantly higher than that of the control group,
and the positive expression of HIF-1α was significantly lower than that of the control group. Conclusion Alprostadil injection
can improve the survival area of venous congestion flaps by promoting the expression of vascular endothelial growth factor in
skin flaps and reducing the expression of HIF-1α in skin flaps, promoting the rate of neovascularization in congested skin flaps
and alleviating the hypoxia and ischemia of congested skin flaps.
